The name of your medicine is Myogesic®. Myogesic® contains orphenadrine citrate and paracetamol as active ingredients.
Orphenadrine citrate is a skeletal muscle relaxant. It acts in the central nervous system to produce muscle relaxant effects.
Paracetamol is used to treat or prevent pain and reduce fever.
Myogesic® is used to treat tension headache and headaches caused by spasm of the muscles in the back of your head and neck.
It is also used to help relax certain muscles in your body and to relieve the pain and discomfort caused by sprains, strains or other injury to your muscles.
Your doctor may have prescribed Myogesic® for another purpose. Ask your doctor if you have any questions about why this medicine has been prescribed for you.
· Do not take Myogesic®
Do not take Myogesic® if you have had an allergic reaction to orphenadrine citrate, paracetamol, other similar medicines, or to any of the ingredients listed at the end of this leaflet.
Do not take Myogesic® if you have any of the following:
• glaucoma (high pressure in the eye);
• intestinal blockage or esophageal disease
• enlarged prostate or bladder obstruction;
• myasthenia gravis (a disease of the muscles causing drooping eyelids, double vision, difficulty in speaking and swallowing, and, sometimes, muscle weakness in the arms or legs).
Do not give this medicine to a child under the age of 12 years because safety and effectiveness in children younger than 12 years have not been established.
· Take special care with Myogesic®
Before you start to take Myogesic® , tell your doctor or pharmacist if:
- you have allergies to any other medicines, foods, preservatives or dyes;
- you have any heart problems;
- you have any liver or kidney problems;
- you have alcohol abuse problems.
- you are taking other Paracetamol containing products.
· Taking other medicines, herbal or dietary supplements
Tell your doctor or pharmacist if you are taking any other medicines, including any that you buy without a prescription from your pharmacy.
Some medicines when taken with Myogesic® may interfere with each other. These include:
• anticholinergics (medicines used to relieve stomach cramps or spasms).
• medicines used to treat depression.
• other central nervous system depressant drugs, including alcohol. Examples of CNS depressants are antihistamines or medicines for hay fever, other allergies or colds; sedatives or tranquilizers; narcotic analgesics (e.g. dextropropoxyphene); barbiturates; medicines for seizures; anaesthetics, including some dental anaesthetics; and other muscle relaxants.
• medicines used to treat epilepsy or fits.
• medicines used to prevent blood clots.
• medicines which affect gastric emptying e.g. metoclopramide, propantheline.
• chloramphenicol, which is an antibiotic
You may need different amounts of your medicines, or you may need to take different medicines. Your doctor or pharmacist has more information on medicines to be careful with or avoid while taking Myogesic®.
· Pregnancy and breast-feeding
Myogesic® is not recommended for use during pregnancy or lactation.
Tell your doctor if you are pregnant or breast feeding or intend to become pregnant or breastfeed.
Your doctor or pharmacist will discuss the possible risks and benefits of using Myogesic® during pregnancy.
If you are not sure whether to start taking this medicine, talk to your doctor.
Ask your doctor, health care provider or pharmacist for advice before taking any medicine.
· Driving and using machines
Be careful driving or operating machinery until you know how the medicine affects you. Myogesic® may cause some people to have blurred vision or to become drowsy, dizzy, light-headed, faint or less alert than they are normally. It may also cause muscle weakness in some people. If you have any of these symptoms do not drive or operate machinery or do anything else that could be dangerous.
Follow all directions given to you by your doctor or pharmacist carefully. They may differ from the information contained in this leaflet.
If you do not understand the instructions on the pack ask your doctor or pharmacist for help.
The usual dose of Myogesic® in adults is two tablets three times a day. Your doctor will prescribe the correct dose for you.
Take Myogesic® tablets only as directed by your doctor or pharmacist. Do not change the dose unless your doctor tells you to do so.
Do not take Myogesic® if the packaging shows signs of tampering.
If you take more Myogesic® than you should
Overdose with paracetamol containing medicines can lead to liver damage.
Do not take more than the recommended dose unless your doctor tells you to.
Immediately contact your doctor or go to an emergency department at the nearest hospital if you think that you or anyone else may have taken too much Myogesic®. Do this even if there are no signs of discomfort or poisoning. You may need urgent medical attention.
Symptoms of an overdose include abdominal pain; vomiting; increased sweating; severe drowsiness; difficulty breathing; cyanosis; excitement; confusion; severe confusion leading to coma; convulsions; fast heart rate; dilated pupils and difficulty passing urine.
If you forget to take Myogesic®
If you miss a dose of this medicine and remember within an hour or so of the missed dose, take it right away. If you do not remember until later, skip the missed dose and go back to taking it as you would normally. Do not take double doses.
Like all medicines, Myogesic® can cause side effects, although not everybody gets them.
All medicines can cause unwanted side effects. Sometimes they are serious, most of the time they are not.
Speak to your doctor if any of the following side effects occur:
Less common: decreased urination; eye pain; fainting; fast or pounding heartbeat.
Rare: hallucinations; shortness of breath; troubled breathing; tightness in chest and/or wheezing; skin rash; sores, ulcers or white spots on lips or in mouth; swollen and/or painful glands; unusual bruising or bleeding; unusual tiredness or weakness.
The following side effects usually do not need medical attention and may go away during treatment as your body adjusts to the medicine.
Speak to your doctor if any of the following side effects continue or are bothersome:
More common: dry mouth.
Myogesic® may cause dryness of the mouth. For temporary relief use sugarless gum or sweets, melt some ice in your mouth or use a saliva substitute. If dry mouth continues for more than 2 weeks, speak to your dentist. Continued dryness of the mouth can increase the chance of dental disease, including tooth decay, gum disease and fungal infections.
Less common or rare: abdominal or stomach cramps or pain; blurred or double vision; confusion; constipation; difficulty in urination; dizziness or light-headedness; drowsiness; excitement, indigestion, irritability, nausea, nervousness or restlessness; headache; muscle weakness; unusually large pupils of the eyes.
Other side effects not listed above may also occur in some patients.
If you notice any side effects that are not listed here tell your doctor or pharmacist.
Keep out of the reach and sight of children.
Protect from light. Store in a dry place. Do not store above 30°C.
Do not take Myogesic® after the expiry date which is printed on the outside of the pack. The expiry
date refers to the last day of that month .
Medicines should not be disposed of via wastewater or household waste. Ask your pharmacist how to
dispose of medicines no longer required. These measures will help to protect the environment.
· What Myogesic® contains
Myogesic® contains orphenadrine citrate and paracetamol as active ingredients. Each tablet contains 35 mg orphenadrine citrate and 450 mg paracetamol.
The other ingredients are: microcrystalline cellulose, hypromellose, sodium starch glycolate, magnesium stearate, colloidal anhydrous silica.

اسم دوائك هو ميوجيسيك. يحتوي ميوجيسيك على أورفينادرين سيترات و باراسيتامول كمواد فعالة.
إن أورفينادرين سيترات مرخي للعضلات الهيكلية. يعمل على الجهاز العصبي المركزي لينتج تأثير مرخي للعضلات.
يستخدم باراسيتامول لعلاج أو الوقاية من الالم وتقليل الحمى.
يستخدم ميوجيسيك لعلاج صداع التوتر والصداع الناتج من تشنج العضلات في مؤخرة الرأس والرقبة.
كما يستخدم للمساعدة على إرخاء عضلات معينة في جسمك و لتخفيف الالم وعدم الراحة الناتج من الالتواء، شد الوتر أو أي ضرر قد يصيب عضلاتك.
قد يصرف لك الطبيب ميوجيسيك لسبب آخر. اذا كان لديك أي استفسارات حول سبب صرف هذا الدواء لك ، إسأل طبيبك.
· موانع استعمال ميوجيسيك
لا تتناول ميوجيسيك اذا كان لديك حساسية مفرطة تجاه أورفينادرين سيترات ، باراسيتامول ، اي ادوية مشابهة أو لأي من المكونات الاخرى المذكورة في نهاية النشرة.
لا تتناول ميوجيسيك اذا كنت تعاني من أي من الحالات التالية:
· الزرق ( ارتفاع ضغط العين).
· انسداد معوي أو مرض في المريء
· تضخم البروستات او انسداد المثانة
· الوهن العضلي ( مرض يصيب العضلات يسبب تدلي الجفون ، الرؤية المزدوجة، صعوبة في الكلام والبلع ، و احيانا ، ضعف في عضلات الذراعين او الساقين).
لا تقم بإعطاء هذا الدواء لطفل أقل من 12 سنة لانه لم يتم تحديد مأمونية وفعالية العلاج في الاطفال دون 12 سنة
· الاحتياطات عند تناول ميوجيسيك
قم باستشارة الطبيب أو الصيدلي قبل تناول ميوجيسيك :
- اذا كنت تعاني من حساسية تجاه اي أدوية أخرى ، أطعمة ، مواد حافظة أو صبغات
- اذا كنت تعاني من مشاكل في القلب
- اذا كنت تعاني من مشاكل في الكلى أو الكبد
- اذا كنت تعاني من مشاكل الإفراط في الكحول
- اذا كنت تتناول اي ادوية أخرى تحتوي على الباراسيتامول
· التداخلات الدوائية من تناول هذا الدواء مع أي أدوية أخرى أو أعشاب أو مكملات غذائية
يرجى إخبار الطبيب أو الصيدلي إذا كنت تتناول أو تناولت مؤخرا أي أدوية أخرى، بما في ذلك الأدوية التي تم الحصول عليها دون وصفة طبية من الصيدلية.
قد تتداخل بعض الادوية مع بعضها اذا تم اعطاؤها بالتزامن مع ميوجيسيك وتشمل :
- مضادات الكولين (الادوية التي تستخدم لعلاج المغص أو تشنجات المعدة )
- الادوية التي تستخدم في علاج الاكتئاب
- مثبطات الجهاز العصبي المركزي بما فيها الكحول. من الامثلة على الادوية مثبطات الجهاز العصبي المركزي : مضادات الهيستامين او الادوية التي تستخدم في علاج حمى القش ، الاشكال الاخرى للحساسية أو نزلات البرد، المنومات أو المهدئات، المسكنات المخدرة ( مثل ديكستروبروبوكسيفين ) ، الباربيتورات ، ادوية علاج الصرع ، ادوية التخدير وتشمل بعض الادوية المخدرة للاسنان و غيرها من مرخيات العضلات.
- الادوية التي تستخدم لعلاج الصرع أوالنوبات
- الادوية التي تستخدم في منع تجلطات الدم
- الادوية التي تؤثر على تفريغ المعدة مثل ميتوكلوبراميد ، بروبانثيلين
- كلورامفينيكول ، مضاد حيوي
قد تحتاج جرعات أخرى من أدويتك أو الى تناول أدوية مختلفة. إن طبيبك أو الصيدلي لديه معلومات اكثر عن الادوية التي يجب تجنبها أو توخي الحذر عند تناولها بالتزامن مع ميوجيسيك.
· الحمل والرضاعة
لا يوصى بتناول ميوجيسيك أثناء الحمل والرضاعة.
أخبري الطبيب اذا كنت حامل أو مرضع. أو تنوين الحمل أو الارضاع.
سيقوم الطبيب أو الصيدلي بمناقشة المخاطر الممكنة والفوائد المرجوة من تناول ميوجيسيك أثناء الحمل.
اسألي طبيبك اذا لم تكوني متأكدا من إمكانية البدء بتناول هذا الدواء.
اطلبي استشارة الطبيب، مقدم الرعاية الصحية أو الصيدلي قبل تناول أي دواء.
· تأثير ميوجيسيك على القيادة واستخدام الآلات
توخى الحذر أثناء القيادة أو استخدام الآلات حتى تعلم تأثير الدواء عليك. قد يشعر بعض الاشخاص بعدم وضوح في الرؤية أو النعاس ، الدوار ،الدوخة ، الاغماء أو تراجع في مدى الانتباه أثناء استخدام ميوجيسيك . كما أنه قد يسبب ضعف في العضلات لدى بعض الاشخاص. اذا ظهرت لديك أي من هذه الاعراض لا تقم بالقيادة أو استخدام الآلات أو اي عمل آخر قد يكون خطيرا.
اتبع جميع التعليمات التي حددها لك الطبيب او الصيدلي بدقة. قد تختلف هذه التعليمات عن المذكورة في هذه النشرة.
اذا لم تكن المعلومات المكتوبة على العلبة مفهومة بالنسبة اليك ، اطلب مساعدة الطبيب أو الصيدلي.
الجرعة الاعتيادية من ميوجيسيك في البالغين هي قرصين ثلاث مرات يوميا. سيحدد الطبيب الجرعة المناسبة لك.
تناول ميوجيسيك حسب ارشادات الطبيب او الصيدلي . لا تقم بتغيير الجرعة ما لم يطلب منك الطبيب ذلك.
لا تتناول ميوجيسيك اذا كان على الغلاف الخارجي علامات عبث.
· الجرعة الزائدة من ميوجيسيك
إن تناول جرعة زائدة من الادوية التي تحتوي على الباراسيتامول قد يسبب تلف في الكبد.
لا تتجاوز الجرعة الموصى بها ما لم يطلب منك الطبيب ذلك.
في حال اعتقادك بأنك تناولت او تناول شخص اخر جرعة زائدة من ميوجيسيك، اتصل بطبيبك على الفور أو قم بزيارة مركز الطوارئ في أقرب مستشفى. قم بذلك حتى لو لم تظهر أي أعراض أو علامات لعدم الشعور بالراحة أو التسمم. قد تحتاج الى عناية طبية طارئة.
تشمل علامات الجرعة الزائدة ألم في البطن ، تقيؤ ، زيادة التعرق، نعاس شديد ، صعوبة في التنفس ، ازرقاق ، انفعال ، ارتباك، ارتباك شديد قد يؤدي الى حدوث غيبوبة ، تشنجات ، سرعة في ضربات القلب ، توسع في بؤبؤ العين و صعوبة في التبول.
نسيان تناول جرعة ميوجيسيك
اذا نسيت تناول جرعة وتذكرت ذلك خلال ساعة ، قم بتناول هذه الجرعة على الفور. اما اذا تذكرت تناول الجرعة بعد مضي وقت أطول ، قم بتجاوز الجرعة الفائتة. واستمر في تناول الجرعات في وقتها المحدد. لا تقم بمضاعفة الجرعات.
شأنه شأن الأدوية الاخرى قد يسبب ميوجيسيك أعراض جانبية . الا أنها لا تحدث عند كل المرضى .
إن جميع الادوية قد تسبب أعراض جانبية غير مرغوب بها ، بعضها خطيرة ، لكن في معظم الاحيان ليست كذلك.
تحدث الى طبيبك اذا كنت تعاني أي من الاعراض الجانبية التالية :
اقل شيوعا: قلة التبول، الم في العين، إغماء ، سرعة في ضربات القلب او زيادة شدتها.
نادرة: هلوسات ، قصر النفس ، اضطراب في التنفس ، ضيق في الصدر و / أو ازيز ، طفح جلدي ، تقرحات ، قرحة أو بقع بيضاء على الشفاه أو في الفم ، تورم و / أو ألم في الغدد ، ظهور كدمات غير اعتيادية أو نزيف ، تعب أو ضعف غير اعتيادي.
الأعراض الجانبية التالية غالبا لا تحتاج الى عناية طبية و قد تختفي أثناء فترة العلاج عندما يتأقلم جسمك مع الدواء.
تحدث الى طبيبك اذا أي من الاعراض الجانبية التالية استمرت أو كانت مزعجة :
أعراض أكثر شيوعا: جفاف الفم .
قد يسبب ميوجيسيك جفاف في الفم. كعلاج مؤقت ، تناول علكة خالية من السكر أو حلويات ، قم بتذويب بعض الجليد في فمك أو استخدم بديل للعاب. اذا استمر جفاف الفم أكثر من اسبوعين ، تحدث الى طبيب اسنانك. قد يزيد جفاف الفم احتمال حدوث مرض في الاسنان يشمل تسوس السن، مرض اللثة و عدوى فطرية.
أعراض أقل شيوعا أو نادرة : مغص أو ألم في البطن أو المعدة ، رؤية غير واضحة أو مزدوجة ، ارتباك ، إمساك، صعوبة في التبول، دوار أو دوخة، نعاس، انفعال، عسر هضم، تهيج، غثيان، عصبية أو قلق ، صداع ، ضعف في العضلات ، توسع غير اعتيادي في بؤبؤ العين.
قد تحدث أعراض جانبية أخرى غير المذكورة في هذه النشرة .
أخبر طبيبك أو الصيدلي اذا لاحظت حدوث أي أعراض جانبية لم يتم ذكرها في هذه النشرة.
يحفظ ميوجيسيك بعيدا عن متناول أيدي الاطفال و نظرهم.
يحفظ بعيدا عن الضوء في مكان جاف على درجة حرارة لا تزيد عن 30 درجة مئوية.
لا تتناول ميوجيسيك بعد تاريخ الانتهاء المذكور على العبوة الخارجية. يدل تاريخ الانتهاء على آخر يوم في الشهر المذكور.
لا تتخلص من الادوية في المياه العادمة او النفايات المنزلية. اسأل الصيدلي حول الطريقة السليمة للتخلص من الادوية التي لم تعد بحاجة اليها. سيساعد هذا في حماية البيئة.
· ماهي محتويات ميوجيسيك
يحتوي ميوجيسيك على أورفينادرين سيترات و باراسيتامول كمواد فعالة . يحتوي كل قرص على 35 ملغم أورفينادرين سيترات و 450 ملغم باراسيتامول.
المكونات الأخرى هي: سيليلوز دقيق البلورية، هيبروميلوز، أملاح الصوديوم لجليكولات النشا، ستيارات المغنيسيوم، سيليكا غروية لامائية.

Myogesic® tablets are indicated for the symptomatic relief of acute painful conditions of the musculo-skeletal system such as low back pain, lumbago, acute strains, sprains, neck pain and myalgia. And it is used to treat tension headache and headaches caused by spasm of the muscles in the back of your head and neck.
Adults: Two tablets three times a day.
Elderly: The cautions apply particularly to elderly patients. Since orphenadrine, in common with anticholinergic effects, may cause confusion (delirium) in the elderly.
It is recommended that a reduced dosage be given.
Children: Not recommended for children under 12 years. Two tablets three times a day for children over 12.
- Orphenadrine citrate should be used with caution in patients with tachycardia, cardiac decompensation, coronary insufficiency or cardiac arrhythmias.
- Paracetamol should be used with caution in patients with hepatic or renal dysfunction.
- Concomitant treatment with other medicines that contain orphenadrine or paracetamol is not recommended.
- Safety of continuous long-term therapy with orphenadrine has not been established. Therefore if orphenadrine is prescribed for prolonged use, periodic monitoring of blood, urine and liver function is recommended.
Interactions have been reported between orphenadrine and phenothiazines and other drugs with anti-muscarinic properties. Concomitant use with alcohol or other CNS depressants should be avoided.
Anticoagulant dosage may require reduction if paracetamol medication is prolonged.
Paracetamol absorption is increased by medicines that increase gastric emptying, e.g. metoclopramide, and decreased by medicines that decrease gastric emptying, e.g. propantheline, antidepressants with anticholinergic properties and narcotic analgesics. Paracetamol may increase chloramphenicol concentration. The likelihood of paracetamol toxicity may be increased by the concomitant use of enzyme inducing agents such as alcohol or anticonvulsant medicines.
Pregnancy and Lactation: There is no evidence as to the drug safety in human pregnancy, nor is there evidence from animal work that it is free from hazard. Avoid in pregnancy unless there is no safer treatment. Orphenadrine and paracetamol are excreted into the breast milk and therefore Myogesic® should not be taken during lactation.
Reaction speed may be lowered during Myogesic® treatment. This must be considered in situations requiring increased care such as driving a vehicle.
Undesirable effects are mainly due to the anti-cholinergic action of orphenadrine and are usually associated with higher doses.
Orphenadrine citrate
More common reactions: the known undesirable effects include; dryness of the mouth, tachycardia, palpitation, urinary hesitancy or retention, blurred vision, dilation of the pupils, increased ocular tension, weakness, nausea, headache, dizziness, constipation and drowsiness. These effects can usually be eliminated by reducing the dose.
Less common reactions: sedation, skin rashes and other allergic reactions are very uncommon undersirable effects. Infrequently an elderly patient may experience some degree of mental confusion. Very rare cases of aplastic anaemia associated with the use of orphenadrine have been reported.
Paracetamol
Reports of adverse reactions are rare. Although the following reactions have been reported, a causal relationship to the administration of paracetamol has been neither confirmed nor refuted; dyspepsia, nausea, allergic and haematological reactions.
No specific information is available on overdose with orphenadrine and paracetamol combination.
Overdose of paracetamol can result in severe liver damage and sometimes acute renal tubular necrosis.
Symptoms and signs
Orphenadrine overdose: known symptoms of overdose with orphenadrine include tachycardia, excitement, confusion and delirium leading to coma. Convulsions, dilated pupils and urinary retention may occur.
Paracetamol overdose: toxic symptoms following an overdose with paracetamol include vomiting, abdominal pain, hypotension, sweating, central stimulation with exhilaration and convulsions in children, drowsiness, respiratory depression, cyanosis and coma.
In adults, hepatotoxicity may occur after ingestion of a single dose of paracetamol 10 to 15g; a dose of 25g or more is potentially fatal.
Symptoms during the first two days of acute poisoning by paracetamol do not reflect the potential seriousness of the intoxication, major manifestations of liver failure such as jaundice, hypoglycaemia and metabolic acidosis may take at least three days to develop.
Treatment
Prompt treatment is essential even when there are no abvious symptoms.
In cases of overdose, methods of reducing absorption of ingested medicine are important.
Prompt administration of activated charcoal 50g in 150ml of water and 150ml sorbitol 50% solution by mouth may reduce absorption. It is recommended that intravenous fluids such as normal saline be given concurrently. Gastric lavage is indicated if the patient is unwilling or unable to drink an activated charcoal/sorbitol mixture.
If the history suggest that paracetamol 150mg/kg body weight or 15g total or more has been ingested, administer the following antidote:
Intravenous acetylcysteine 20%: administer acetylcysteine immediately without waiting for positive urine test or plasma level results if 8 hours or less since overdose ingestion. Initial dose 150mg/kg over 15 minutes, followed by continuous infusion of 50mg/kg in glucose 5% 500ml over four hours and 100mg/kg in glucose 5% 1L over 16 hours. If more than eight hours have elapsed since the overdose was taken, the antidote may be less effective.
Convulsions and delirium respond to relatively large doses of diazepam, preferably by mouth. Adequate hydration of the patient is important.
Pharmacotheraputic group: Analgesic and skeletal muscle relaxant.
Orphenadrine acts centrally by blocking reticular facilitation i.e. it blocks preferentially those pathways whose hyperactivity leads to exaggeration of motor function such as spasticity, rigidity or muscle-spasm.
Paracetamol is a well-tolerated analgesic and antipyretic.
Its analgesic action is rapid in onset (within 30 minutes) and lasts for three to four hours. It has been found to be particularly effective for the relief of pain muscles and joints. ATC code: M03BC51
Myogesic® is well absorbed from the gastrointestinal tract. Orphenadrine is rapidly distributed in the body tissues; most of the dose is metabolized and excreted in the urine. Paracetamol is readily absorbed from the gastrointestinal tract. It diffuses throughout the body tissues, and is excreted fairly rapidly in the urine.

Myogesic® Tablets: Packs of 20, 50, 300 and 500 tablets.
Myogesic® tablets are available in packs of 20 tablets (2 blisters of 10 tablets), 50 tablets (5 blisters of 10 tablets), 300 tablets (30 blisters of 10 tablets) and 500 tablets (50 blisters of 10 tablets).
Not all pack sizes may be marketed
Any unused product or waste material should be disposed of in accordance with local requirements.
