Dramylin Paediatric is a medicine used to relieve the symptoms of cough and congestion. It also helps relieve runny nose and sneezing associated with hayfever and other allergies. The syrup contains diphenhydramine hydrochloride which is an antihistamine that helps relieve coughing and dry secretions in the nose and chest, and menthol which helps you breathe more easily by relieving congestion.
The medicine is for use in children aged 6 to 12 years.
This medicine is suitable for most children but a few children should not use it. If you are in any doubt, talk to your doctor or pharmacist.
Do not give your child this medicine if your child
§ Has ever had a bad reaction to this product or any of the ingredients.
§ Suffers from prostate problems (difficulty with passing water or needing to pass water often).
§ Suffers from or has had closed angle glaucoma (increased pressure in the eye).
§ Is taking, or has taken in the last two weeks, drugs for depression known as Monoamine Oxidase Inhibitors (MAOIs).
§ Is taking any other cough and cold medicine
§ Is aged less than 6 years old.
If any of these apply, get advice from a doctor or pharmacist.
Talk to your doctor or pharmacist If your child
§ Suffers from liver or kidney problems.
§ Has difficulty passing water (urinary retention).
§ Has a persistent cough such as occurs with asthma, has asthma, is suffering from an asthma attack or a cough that produces a lot of mucus (phlegm).
§ Is taking any other medicines containing codeine (sometimes used as a pain killer).
§ Is taking any other medicines, including:
v Antimuscarinic / anticholinergic drugs e.g.:
- tricyclic antidepressants (drugs to treat mood disorders).
- atropine (used to treat some eye conditions and occasionally bowel conditions).
v CNS depressants e.g.:
- barbiturates (a kind of sleeping tablet).
- minor tranquillisers (drugs used to relax muscles or decrease anxiety).
- opioid analgesics (drugs used to relieve pain e.g. codeine, tramadol, morphine).
- antipsychotics (drugs used to treat mood disorders).
If you are not sure about any of the medicines your child is taking, show the bottle or pack to your pharmacist.
If any of these bullet points apply now or in the past, talk to a doctor or pharmacist.
Pregnancy and breast feeding
Ask your doctor or pharmacist for advice before taking this medicine if you are pregnant or breast-feeding.
Driving and operating machines
§ This product can make some people feel drowsy, dizzy or have blurred vision. Make sure that your child is not affected if he or she is going to do anything where they need to be alert. Be especially careful if the child is taking another medicine which may also cause drowsiness.
§ If an older child or adult is taking the medicine remember that this product may cause drowsiness, dizziness or blurred vision.
Do not drive or operate machinery if you are affected. Avoid alcoholic drink.

Children under 6 years
Do not give to children under 6 years old.
Children 6 – 12 years:
Two 5 ml spoonful every 6 hours
§ Do not give more than 4 doses in 24 hours.
§ Do not use for more than 5 days without the advice of a doctor.
§ Do not use to sedate a child.
§ If symptoms persist or worsen, talk to your doctor or pharmacist.
If anyone has too much
Contact a doctor or your nearest Accident and Emergency Department (Casualty) taking this leaflet and pack with you.
If you forget to give the medicine
You should only give this medicine as required following the dosage instructions above carefully. If you forget to give a dose, give the next dose when needed provided that the last dose was at least 6 hours ago. Do not give a double dose.
Dramylin Paediatric syrup can have side-effects, like all medicines, although these don’t affect everyone and are usually mild.
If your child experiences any of the following, stop using the medicine and seek immediate help:
§ Allergic reactions including skin rashes (which may be severe and include blistering and peeling of the skin) and itching.
§ Rarely people can experience fits.
§ Rarely abnormal blood test results for liver function.
If your child experiences any of the following, stop using the medicine and talk to your doctor:
§ Rarely people can have an irregular heartbeat or an increased awareness of the heartbeat (palpitations).
§ Sleep disturbances.
Other effects that may occur include:
§ Commonly: drowsiness, including some difficulty performing tasks.
§ Commonly: headache, upset stomach, difficulty in passing urine, stimulation, blurred vision, dry mouth, thickened mucus.
§ Rarely: low blood pressure, tremor, shakiness, slurred speech, dizziness, confusion, depression.
§ Rarely: blood disorders.
If this side effect gets serious, or if you notice any side effects not listed in this leaflet, please tell your doctor or pharmacist.
§ Keep in a safe place, out of the reach and sight of children.
§ Store below 30° C.
§ Use within 3 months after opening
§ Do not use Dramylin Paediatric Syrup after the expiry date printed on the packaging. The expiry date refers to the last day of the month.
Medicines should not be disposed of via wastewater or household waste. Ask your pharmacist how to dispose of medicines that are no longer required. These measures will help to protect the environment.
What Dramylin Paediatric Syrup contains
The active substances in each 5 ml are:
Diphenhydramine HCl: 7.0 mg.
Menthol: 0.55 mg.
The excipients are:
Methyl Paraben, Propyl Paraben, Sucrose, Citric Acid Monohydrate, Sodium Calcium Edetate, Propylene Glycol, Sodium citrate, Glycerol 99.5%, Sunset Yellow Color, Essence of Banana, Essence of Orange and Purified Water.

درامیلین للأطفال هو دواء يستخدم لتخفيف أعراض السعال و الاحتقان. يساعد الدواء أيضًا على تخفيف رشح الأنف و العطس المرتبط بالحمى و أنواع الحساسية الأخرى. يحتوي الشراب على دیفینھیدرامین ھیدروكلورید و هو مضاد للحساسية يساعد في تخفيف السعال و الإفرازات الجافة في الأنف و الصدر، و المينثول يساعدك على التنفس بسهولة عن طريق تخفيف الاحتقان. يستخدم هذا الدواء للأطفال من عمر ٦ – ١۲ عام.
إن هذا الدواء مناسب لمعظم الأطفال لكن لا يجب استخدامه للقليل منهم. إذا راودك أي شك، تحدث إلى طبيبك أو الصيدلي.
لا تعطي هذا الدواء لطفلك إذا كان الطفل:
§ عانى من قبل حساسية من هذا المنتج أو أي من مكوناته.
§ يعاني من مشاكل بالبروستاتا (صعوبة في التبول أو شعور بالاحتياج للتبول أحيانًا كثيرة)
§ يعاني أو عانى من قبل جلوكوما الزاوية المغلقة (زيادة ضغط العين)
§ يتناول أو تناول في الأسبوعين الماضيين، دواء لعلاج الاكتئاب يدعى مثبطات أكسیدیز أحادي الأمين
§ يتناول أي أدوية أخرى لعلاج البرد و السعال
§ عمره أقل من ٦ سنوات
إذا انطبق أي من ذلك على طفلك، استشر الطبيب أو الصيدلي.
تحدث إلى طبيبك أو الصيدلي إذا كان طفلك
§ يعاني من اعتلال كبدي أو كلوي
§ يعاني صعوبة في التبول (احتباس بولي)
§ لديه سعال مستمر مثل الذي يحدث مع الربو، أو لديه ربو أو يعاني من نوبة ربو أو يعاني من السعال الذي ينتج الكثير من المخاط (البلغم)
§ يتناول أي دواء آخر يحتوي على كودايين (يستخدم كمسكن شديد للألم)
§ يتناول أي من الأدوية التالية:
o الأدوية مضادات الكولین / مضادات المسكارین
- مضادات الاكتئاب ثلاثية الحلقات (تعالج اضطرابات المزاج)
- أتروبین (يستخدم لعلاج أمراض العين و أحيانًا اضطرابات الأمعاء)
o مثبطات الجهاز العصبي المركزي
- باربیتیورات (نوع من الأدوية التي تساعد على النوم)
- المهدئات الثانوية (الأدوية المستخدمة لإرخاء العضلات أو تقليل القلق)
- المسكنات الأفيونية (الأدوية المستخدمة لتخفيف الألم مثل كودایین، ترامادول و المورفین)
- مضادات الذهان (الأدوية المستخدمة لعلاج اضطرابات المزاج)
إذا كنت غير متأكدًا من أي من الأدوية التي يتناولها طفلك، قم بعرض الزجاجة أو العبوة على الصيدلي.
إذا كان أي من هذه النقاط تنطبق على طفلك الآن أو في الماضي، تحدث الطبيب أو الصيدلي.
الحمل و الرضاعة الطبيعية
استشيري الطبيب أو الصيدلي قبل تناول هذا الدواء إذا كنتِ حاملاً أو أماً مرضعة.
القيادة و استخدام الآلات
§ إن هذا المنتج يجعل بعض الناس يشعرون بالنعاس، الدوار أو عدم وضوح الرؤية. تأكد من أن طفلك لا يصيبه هذا التأثير إذا كان موشك على عمل أي شيء يحتاج منه التيقظ. كن حذراً خاصة إذا كان طفلك يتناول دواء آخر قد يسبب له النعاس
§ إذا تناول طفل أكبر سنًا أو بالغ هذا الدواء عليك أن تتذكر أن هذا الدواء ربما يسبب النعاس أو الدوخة أو عدم وضوح الرؤية. لا تقم بقيادة السيارة أو تشغيل الآلات إذا أصابك هذا التأثير.
اجتنب تناول الكحول
- يستخدم فقط عن طريق الفم.
- لا تستخدم أكثر من الجرعة الموضحة أدناه
الأطفال دون سن ٦ سنوات: لا يستخدم للأطفال دون سن ٦ سنوات.
الأطفال من ٦ - ۱۲ سنة: ملعقتين صغيرتين (٥ مل) كل ٦ ساعات.
§ لا تعطي الطفل أكثر من أربع جرعات يوميًا.
§ لا تستخدمه لأكثر من خمس أيام دون نصيحة الطبيب
§ لا تستخدمه كمهدئ للطفل
§ إذا استمرت الأعراض أو زادت الحالة سوءاً، تحدث إلى طبيبك أو الصيدلي.
إذا تناول أي شخص الكثير
اتصل بالطبيب أو قسم الطوارئ في أقرب مستشفى وخذ هذه النشرة و العبوة معك.
إذا نسيت إعطاء الدواء لطفلك
عليك إعطاء هذا الدواء فقط كما هو مطلوب، متبعًا بعناية تعليمات الجرعة المذكورة أعلاه. إذا نسيت إعطاء طفلك جرعة، قم بإعطائه الجرعة التالية عند الحاجة بشرط مرور ٦ ساعات على الأقل على تناول آخر جرعة. لا تقم بإعطاء الطفل جرعة مضاعفة.
كما في جميع الأدوية قد يسبب درامیلین للأطفال تأثيرات جانبية و مع ذلك لا تحدث مع جميع الأشخاص و غالبًا ما تكون بسيطة.
إذا عانى طفلك أي من التالي، توقف عن إعطاؤه الدواء و اطلب المساعدة الطبية فوراً.
§ تفاعلات حساسية تشمل طفح جلدي (و الذي ربما يكون حاد و يشمل ظهور بثور و تقشير الجلد) و حكة.
§ نادرًا ما يصاب البعض بتشنجات.
§ نادرًا ما تصبح نتائج فحص الدم لوظائف الكبد غير طبيعية.
إذا أصاب طفلك أي من التالي، توقف عن إعطاؤه الدواء و تحدث إلى الطبيب:
§ نادرًا ما يصيب البعض اضطراب في نظم القلب أو زيادة الشعور بضربات القلب (خفقان)
§ اضطرابات النوم
الأعراض الجانبية الأخرى المحتملة الحدوث تشمل:
§ شائعة: النعاس، بما في ذلك مواجهة بعض الصعوبة في أداء المهام.
§ شائعة: صداع، اضطراب المعدة، صعوبة في التبول، الأثارة، عدم وضوح الرؤية، جفاف الفم وجود مخاط سميك.
§ نادرة: انخفاض ضغط الدم، رعشة، اهتزاز، ثقل في اللسان، دوخة، ارتباك و اكتئاب.
§ نادرة: اضطرابات بالدم.
يرجى إخبار الطبيب إذا أصبحت إحدى تلك الآثار الجانبية خطيرة أو إذا لاحظت حدوث آثار جانبية غير مذكورة في هذه النشرة
§ احفظ الدواء في مكان آمن بعيدًا عن متناول و نظر الأطفال.
§ يحفظ في درجة حرارة أقل من ۳۰ درجة مئوية.
§ يستخدم في خلال ۳ أشهر من فتح العبوة.
§ لا تستخدم شراب درامیلین للأطفال بعد تاريخ انتهاء الصلاحية المطبوع على العبوة. تاريخ الانتهاء يشير إلى اليوم الأخير من الشهر.
§ يجب أن لا يتم التخلص من الأدوية عن طريق مياه الصرف الصحي أو النفايات المنزلية. اسأل الصيدلي عن كيفية التخلص من الأدوية التي لم تعد مطلوبة. و سوف تساعد هذه التدابير على حماية البيئة.

Dramylin paediatric Syrup is indicated for the relief of cough and its congestive symptoms, runny nose and sneezing, and in the treatment of hay fever and other allergic conditions affecting the upper respiratory tract.
Route of Administration: Oral
Children under 6 years:
Dramylin paediatric Syrup is contraindicated in children under the age of 6 years (see section 4.3).
Children 6 to 12 years:
Two 5 ml spoonfuls every 6 hours
No more than four doses should be given in any 24 hours.
Not to be used for more than five days without the advice of a doctor. Parents or carers should seek medical attention if the child's condition deteriorates during treatment.
Do not exceed the stated dose.
Keep out of the reach and sight of children.
· Diphenhydramine should not be taken by patients with susceptibility to angle-closure glaucoma or symptomatic prostatic hypertrophy unless directed by a doctor.
· Alcohol or other potential sedating medicines should not be used concurrently with Dramylin Paediatric syrup
· Patients with hepatic or moderate to severe renal dysfunction or urinary retention should exercise caution when using this product (see Pharmacokinetics - Renal/Hepatic Dysfunction).
The product may cause drowsiness. This product should not be used to sedate a child.
Do not exceed the stated dose.
Do not take with any other cough and cold medicine.
Ask a doctor before use if your child suffers from a chronic or persistent cough, if he/she has asthma, is suffering from an acute asthma attack or where cough is accompanied by excessive secretions
· CNS depressants: may enhance the sedative effects of CNS depressants including barbiturates, hypnotics, opioid analgesics, anxiolytic sedatives, antipsychotics and alcohol.
· Antimuscarinic drugs: may have an additive muscarinic action with other drugs, such as atropine and some antidepressants.
· MAOIs: Not to be used in patients taking MAOIs or within 14 days of stopping treatment as there is a risk of serotonin syndrome
Diphenhydramine crosses the placenta and has been detected in breast milk. Dramylin Paediatric syrup should only be used when the potential benefit of treatment to the mother exceeds any possible hazards to the developing foetus or suckling infant.
This preparation may cause drowsiness, dizziness or blurred vision. If affected, the patient should not drive or operate machinery
Common side effects:
· CNS effects: Drowsiness (usually diminishes within a few days), paradoxical stimulation, headache, psychomotor impairment.
· Antimuscarinic effects: Urinary retention, dry mouth, blurred vision, gastrointestinal disturbances, thickened respiratory tract secretions.
Rare side effects:
Hypotension, extrapyramidal effects, dizziness, confusion, depression, sleep disturbances, tremor, convulsions, palpitation, arrhythmia, hypersensitivity reactions, blood disorders and liver dysfunction
Adverse reactions to menthol at the low concentration present in Dramylin Paediatric syrup are not anticipated

Signs and Symptoms:
Drowsiness, hyperpyrexia and anticholinergic effects. In children, CNS excitation, including hallucinations and convulsions may appear; with larger doses, coma or cardiovascular collapse may follow.
Treatment
Treatment of overdose with Dramylin Paediatric syrup is likely to involve supportive care and rapid gastric emptying with Syrup of Ipecac induced emesis or gastric lavage. In cases of acute poisoning, activated charcoal may be useful. Seizures may be controlled with Diazepam or Thiopental Sodium. In addition to supportive care, the intravenous use of Physostigmine may be efficacious in antagonising severe anticholinergic symptoms

Mode of Action of each ingredient:
Diphenhydramine is a potent antihistamine and antitussive with anticholinergic properties. Recent experiments have shown that the antitussive action is discrete from H1-receptor blockade and is located in the brain stem.
Menthol has mild local anaesthetic and decongestant properties.
Diphenhydramine is well absorbed from the gastrointestinal tract. Peak serum levels are reached at between 2-2.5 hours after an oral dose. Duration of activity is between 4 - 8 hours. The drug is widely distributed throughout the body, including the CNS, and some 78% is bound to plasma proteins. Estimates of the volume of distribution lie in the range 3.3 - 6.8 l/kg.
Diphenhydramine experiences extensive first-pass metabolism, undergoing two successive N-Demethylations; the resultant amine is then oxidised to a carboxylic acid. Values for plasma clearance lie in the range 600 - 1300 ml/min and the terminal elimination half life lies in the range 3.4 - 9.3 hours. Little unchanged drug is excreted in the urine.
Pharmacokinetic studies in elderly subjects indicate no major differences in drug distribution or elimination compared with younger adults.
Menthol: After absorption, menthol is conjugated in the liver and excreted both in urine and bile as the glucuronide
Renal Dysfunction
The results of a review on the use of diphenhydramine in renal failure suggest that in moderate to severe renal failure, the dose interval should be extended by a period dependent on Glomerular filtration rate (GFR).
Hepatic Dysfunction
After intravenous administration of 0.8 mg/kg diphenhydramine, a prolonged half-life was noted in patients with chronic liver disease which correlated with the severity of the disease. However, the mean plasma clearance and apparent volume of distribution were not significantly affected
