Hyopan Syrup is an anticholinergic agent exerts a spasmolytic action on the smooth muscle of the gastrointestinal, genito-urinary & biliary tracts by blocking the muscarinic receptors in the viscera.

HYOPAN Syrup is used for:

·         Gastrointestinal spasms.

·         Spasms of urinary tract.

·         Biliary dyskinesia.

·         Dysmenorrhoea and delayed relaxation of the lower uterine segment.

·         Postoperative vomiting.

Pylorospasm in infants.

The usual dose is as follows, to be taken orally:

Adults and children over 6 years:

1-2 tablespoonful (10-20 ml), 3-5 times daily.

Children 1-6 years:

1-2 teaspoonful (5-10 ml), 3 times daily.

Children under 1 year:

1 teaspoonful (5ml), 3 times daily.

Hyopan should be used with caution in cardiac diseases, hypertension, glaucoma, in liver and kidneys dysfunction and in elderly patients.

Do not exceed the stated dose.

1.      The anticholinergic side effects of tricyclic antidepressants & antihistamines may be increased when given concomitantly with Hyopan.

Co-administration of Hyopan with dopamine antagonists such as metoclopramide diminishes the effect of both drugs in the gastrointestinal tract.

No negative symptoms have been observed for using Hyopan in a pregnant or lactating women.

However, it should be avoided in the first trimester.

Because of possible visual accommodation disturbances patients should not drive or operate machinery if affected.

·         Anticholinergic side effects include: dry mouth, constipation, blurred vision, tachycardia & urinary retention.

·         In rare cases hypersensitivity reactions such as urticaria.

Hyoscine- N- Butyl Bromide is less lipid soluble than atropine preparations and is less likely to cross blood brain barrier, therefore anticholinergic side effects at central nervous system such as drowsiness, headache & confusion are not observed.

To report any side effect(s):

In case of overdose the anticholinergic effects will occur such as: dry mouth, tachycardia, urinary

retention and blurred vision may occur within 5 hours of the ingestion.

In such case, gastric lavage using medical charcoal followed by 15% magnesium sulphate is used.

As a quaternary ammonium derivative, hyoscine butylbromide does not enter the central nervous system. Therefore, anticholinergic side effects at the central nervous system do not occur. Peripheral anticholinergic action results from a ganglion-blocking action within the visceral wall as well as from an anti-muscarinic activity.

ABSORPTION:

Hyoscine-N-Butyl Bromide is a quaternary ammonium compound with high polarity. Hence, partially absorbed after oral administration (8%). Its systemic bioavailability is less than 1%, despite the briefly low blood levels, Hyoscine-N-Butyl Bromide has relatively high local concentrations at site of action in the gastrointestinal tract, gallbladder, kidneys and liver.

DISTRIBUTION:

Because of its high affinity for muscarinic receptors and nicotinic receptors, hyoscine butylbromide is mainly distributed on muscle cells of the abdominal and pelvic area as well as in the intramural ganglia of the abdominal organs. Plasma protein binding (albumin) of hyoscine butylbromide is approximately 4.4%. It does not cross the blood brain barrier.

METABOLISM AND ELIMINATIO:

It is metabolized in the liver & excreted in the urine as a metabolite compounds.
