Hymox is antibiotic. It contains a medicine called Amoxicillin. This belongs to a group of medicines called Penicillins.

WHAT IT IS USED FOR

Hymox can be used to treat a variety of infections in different parts of the body caused by bacteria. It is also used to stop infections when you have a tooth removed or other dental surgery. Hymox capsules may be used in combination with other medicines to treat stomach ulcers.


Do not take Hymox if:

•  You are allergic (hypersensitive) to amoxicillin, penicillin or any of the other ingredients of Hymox (listed in section 6).

•  You have ever had an allergic (hypersensitive) reaction to any antibiotic. This can include a skin rash or swelling of the face or neck.

Do not take this medicine if any of the above applies. If you are not sure, consult your doctor or pharmacist before taking Hymox.

 

Take special care with Hymox

Check with your doctor or pharmacist before taking Hymox, if:

•  You have glandular fever

•  You suffer from kidney disease

•  If you take Hymox for a long time, this may lead to an increase in the growth of bacteria that will become resistant to the antibiotic

•  If used in premature or newborn babies. Kidney, liver and blood functions should be monitored.

•  You are not passing water regularly

Abnormal prolongation of prothrombin time (increased INR) has been reported rarely in patients receiving amoxicillin and oral anticoagulants. Appropriate monitoring should be undertaken when anticoagulants are prescribed concurrently. Adjustments in the dose of oral anticoagulants may be necessary to maintain the desired level of anticoagulation.

 

• Severe Cutaneous Adverse Reactions:

Amoxicillin may cause severe cutaneous adverse reactions (SCAR), such as Stevens-Johnson syndrome (SJS), toxic epidermal necrolysis (TEN), drug reaction with eosinophilia and systemic symptoms (DRESS), and acute generalized exanthematous pustulosis (AGEP). If patients develop a skin rash they should be monitored closely and amoxicillin discontinued if lesions progress.

If you are not sure if any of the above apply to you, talk to your doctor or pharmacist.

Having urine or blood tests

If you are having tests on your water (urine glucose tests) or blood tests for liver function, let the doctor or nurse know that you are on Hymox. Hymox can affect the results of these tests.

Taking other medicines

Please tell your doctor or pharmacist if you are already taking any other medicines, including medicines obtained without prescription. This is because Hymox can affect the way some other medicines work. Also some medicines can affect the way Hymox works.

–  In the literature there are rare cases of increased international normalised ratio in patients maintained on acenocoumarol or warfarin and prescribed a course of amoxicillin. If co- administration is necessary, the prothrombin time or international normalised ratio should be carefully monitored with the addition or withdrawal of amoxicillin (see 2. Before you take Hymox).

–  If you are taking allopurinol (used for gout) with Hymox, it may be more likely that you will have an allergic skin reaction.

–  If you are taking Probenecid (to prevent gout). Your doctor may decide to adjust your dose of Hymox.

–  If medicines to help stop blood clots (such as warfarin) are taken with Hymox then extra blood test may be needed.

–  If you are taking Oral Contraceptives, you will need to use extra contraceptive precautions, such as using a condom.

If you need any advice, talk to your doctor or pharmacist.

Pregnancy and breast feeding

Ask your doctor or pharmacist for advice before taking this medicine. If you think you might be or if you are pregnant, or if you are breast-feeding.

Driving and using machines

Hymox can have side effects and the symptoms may make you unfit to drive. Don’t drive or operate machinery unless you are feeling well.


Always take Hymox exactly as your doctor has told you. If you are not sure, check with your doctor or pharmacist

It is important that you complete the course of treatment as directed by your doctor.

·       Hymox should be taken orally (by mouth) only.

·       Swallow the capsules or tablets whole with a glass of water at the start of a meal or slightly before.

·       Space the doses evenly during the day, at least 4 hours apart.

·       Never take 2 doses in 1 hour

·       The maximum recommended dose for adults and children over 40 kg is 6 g per day given as 2x3 g doses. While the maximum recommended dose for children weighing less than 40 kg is 3 grams.

·       For young children (less than 6 years old), one of the infant formulas of this age should be used.

The usual dose is:

Children weighing less than 40 kg who are able to swallow capsules

All doses are worked out depending on the child’s body weight in kilograms.

·       Your doctor or pharmacist will advise you how much Hymox capsules or tablets you should give to your child.

·       Children weighing 40 kg or more should be prescribed the adult dosage.

·       Recommended doses:

50 mg/kg, combined with 25 mg/kg probenecid as a single dose.

note: since probenecid is

contraindicated in children under 2 years, do not use this regimen in

these cases.

Lyme disease

Early stage: 25 to 50 mg/kg/day in three divided doses for 10 to 21 days

Late stage (systemic involvement):

100 mg/kg/day in three divided doses for 10 to 30 days

+ Consideration should be given to the official treatment guidelines for each indication.

*Twice daily dosing regimens should only be considered when the dose is in the upper range.

 

·       To prevent dental infection – a single dose of 50 mg per kilogram of body weight is given before dental surgery.

 

 

Adults, Elderly Patients and Children weighing more than 40kg:

Acute otitis media

500 mg every 8 hours, 875 mg every 12 hours

For severe infections 875 mg every 8 hours for 10 days

Acute streptococcal tonsillitis and pharyngitis

Acute exacerbations of chronic bronchitis

Community acquired pneumonia

500 mg to 1 g every 8 hours

Typhoid and paratyphoid fever

500 mg to 2 g every 8 hours

Prosthetic joint infections

500 mg to 1 g every 8 hours

Prophylaxis of endocarditis

2 g orally, single dose 30 to 60 minutes before procedure

Stomach ulcers (Helicobacter pylori eradication):

1 x 750 mg dose (3 x 250 mg capsules or 1 x 500 mg capsule and 1 x 250 mg capsule) or 1 x 1 g dose (2 x 500 mg capsules) twice a day in combination with a proton pump inhibitor (e.g. omeprazole, lansoprazole) and another

antibiotic (e.g. clarithromycin, metronidazole) for 7 days

Lyme disease

Early stage: 500 mg to 1 g every 8 hours up to a maximum of 4 g/day in divided doses for 14 days (10 to 21 days)

Late stage (systemic involvement): 500 mg to 2 g every 8 hours up to a maximum of

6 g/day in divided doses for 10 to 30 days

To stop infection during surgery

–      The dose will vary according to the type of surgery. Other medicines may also be given at the same time.

 

–        Your doctor, pharmacist or nurse can give you more details.

If you have kidney or liver problems:

–        If you suffer from kidney problems, you may need to reduce the dose. Your doctor may choose a different concentration or medication for you.

–        If you suffer from liver problems, you may need to do extensive blood tests to determine how the liver is working.

If you take more than you should

If you (or someone else) swallow a lot of Hymox at the same time, or you think a child may have swallowed Hymox capsules, contact your nearest hospital casualty department or tell your doctor immediately.

Signs might be an upset stomach (feeling sick, being sick or diarrhea) or crystals in the urine which may be seen as cloudy urine, or problems passing urine. Take the medicine to show the doctor.

If you forget to take a dose

If you forget to take a dose do not worry, take it as soon as you remember and carry on as before. Try to wait about four hours before taking the next dose. Do not take a double dose to make up for a forgotten dose.

If you stop taking Hymox

Keep taking Hymox until the treatment is finished, even if you feel better. You need every dose to help fight the infection. If some bacteria survive they can cause the infection to come back. Treatment should be continued for 2 to 3 days after the symptoms have gone.

Do not take Hymox for more than 2 weeks. If you still feel unwell you should go back to see the doctor.

Thrush (a yeast infection of moist areas of the body) may develop if Hymox is used for a long time. If this occurs and you have been taking Hymox for longer than recommended, tell your doctor.

If you have any further questions on the use of this product, ask your doctor or pharmacist.


Like all medicines, Hymox can cause side effects, although not everybody gets them. Please tell your doctor or pharmacist if you notice any of the following effects or any effects not listed. The following side effects may happen with this medicine.

Stop taking Hymox and see your doctor straight away, if you notice any of the following serious side effects- you may need urgent medical treatment:

The following are very rare (affects less than 1 in 10,000 people)

·       Hypersensitivity or severe allergic reaction including itchy rash, itching, sore mouth or eyes, swelling of the face, lips, throat or tongue or breathing difficulties. These can be serious and occasionally deaths have occurred.

·       Rash or pinpoint flat red round spots under the skin surface or bruising of the skin. This is a result of an allergic reaction caused by the inflammation of blood vessel walls (vasculitis) due to an allergic reaction. This can be associated with joint pain (arthritis) and kidney problems.

·       A delayed allergic reaction can occur 7 to 12 days after taking Hymox, some signs include: rashes, fever, joint pains and enlargement of the lymph nodes especially under the arms.

 

·       A skin reaction called ‘erythema multiforme’: signs including itchy reddish-purple patches on the skin especially on the palms of the hands or soles of the feet, hive-like raised swollen areas on the skin, tender area on the mouth, eyes and private parts. You may have a fever and be very tired.

·       Other severe skin reactions such as change in skin colour, bumps under the skin, blistering, peeling, redness, pain, scaling. These may be associated with fever, headaches and body aches.

·       High temperature (fever), chills, a sore throat or other signs of an infection, or if you bruise easily. These may be signs of a problem with your blood cells.

·       Serious liver side effects may occur which are reversible. They are mainly associated with people having treatment over a long period, males and the elderly. You must tell your doctor urgently if you get:

o   severe diarrhoea with bleeding

o   blisters, redness or bruising of the skin

o   darker urine or paler stools

o   yellowing of the skin or the white of the eyes (jaundice). See also anaemia below which might result in jaundice.

These can happen up to several weeks after the treatment.

·       Hepatitis

·       Inflammation of the large bowel (colon), with diarrhoea sometimes containing blood, pain and fever.

·       fits (convulsions), seen in patients on high dose or kidney problems

If any of the above happen stop taking the medicine and see your doctor straight away.

Sometimes you may get less sever skin reactions such as:

–    a mildly itchy rash (round, pink red patches). “hive-like” swollen areas on firearms, legs, palms, hands or feet. This is uncommon (affect less than 1in 100 people).

If you have any of these talk to your doctor as Hymox will need to be stopped.

Other possible side effects are:

Common (affects less than 1 in 10 people)

·       skin rash

·       feeling sick (nausea)

·       diarrhea

Uncommon (affects less than 1 in 100 people)

·       being sick (vomiting)

·       Itching and rash.

Very rare (affects less than 1 in 10,000 people)

·       thrush (a yeast infection of the vagina, mouth or skin folds), you can get treatment for thrush from your doctor or pharmacist

·       kidney problems

·       dizziness

·       hyperactivity

·       crystals in the urine, which may be seen as cloudy urine, or difficulty in passing urine. Make sure you drink plenty of fluids to reduce the chance of these symptoms.

·       teeth may appear stained, usually returning to normal with brushing (this has been reported in children)

 

·       tongue may change to yellow, brown or black and it may have a hairy appearance

·       an excessive breakdown of red blood cells causing a form of anaemia. Signs include tiredness, headaches, shortness of breath, dizziness, looking pale and yellowing of the skin and the whites of the eyes

·       blood may take longer to clot than it normally would. You may notice this if you have a nosebleed or cut yourself.

·       Skin reactions such as erythema multiforme, Stevens-Johnson Syndrome, toxic epidermal necrolysis, bullous and exfoliative dermatitis and acute generalised exanthematouspustulosis (AGEP), and drug reaction with eosinophilia and systemic symptoms (DRESS).

If any of the side effects gets serious or if you notice any side effects not listed in this leaflet. Please tell your doctor or pharmacist.


 

·       Keep out of the reach and sight of children.

·       Store in dry place, below 25°C

·       Do not take Hymox after the expiry date stated on the pack after EXP. The expiry date refers to the last day of the month.

·       Medicines should not be disposed of via wastewater or household waste. Ask your pharmacist how to dispose of medicines no longer required. These measures will help protect the environment


–      The active substance is Amoxicillin.

Each 250 mg capsule contains 250mg amoxicillin Each 500 mg capsule contains 500mg amoxicillin

Each 875 mg film coated tablet contains 875mg amoxicillin

–      The other ingredients are:

Hymox 250mg, 500mg capsules: Magnesium Stearate, Gelatin Capsule No. 2, Gelatin Capsule No. 0el

Hymox 875mg film coated tablets: Avicel PH 102, Crospovidone NF, Sodium Starch Glycolate, Colloidal Silicon Dioxide, Magnesium Stearate and Opadry Pink YS-1-14725


Hymox 250mg capsules: Size No. 2, Ivory hard gelatin capsules fitted with scarlet caps containing a white to off white powder. The capsules are printed with “HYMOX 250” on the body and cap. Each pack of Hymox 250mg capsules contains 20 capsules Hymox 500mg capsules: Size No. 0el, Ivory hard gelatin capsules fitted with scarlet caps containing a white to off white powder. The capsules are printed with “HYMOX 500” on the body and cap. ﻻ ﺗﺘﻨﺎول ھﺬا اﻟﺪواء إذا ﻛﺎن أى ﻣﻤﺎ ﺳﺒﻖ ﯾﻨﻄﺒﻖ ﻋﻠﯿﻚ. إذا ﻛﻨﺖ ﻏﯿﺮ ﻣﺘﺄﻛﺪ اﺳﺘﺸﺮ طﺒﯿﺒﻚ أو اﻟﺼﯿﺪﻟﻰ ﻗﺒﻞ ﺗﻨﺎول ھﺎﯾﻤﻮﻛﺲ.

 

ﯾﻨﺒﻐﻰ ﺗﻮﺧﻰ اﻟﺤﺬر ﻣﻊ ھﺎﯾﻤﻮﻛﺲ ﻓﻰ اﻟﺤﺎﻻت اﻟﺘﺎﻟﯿﺔ

ﺗﺄﻛﺪ ﻣﻦ طﺒﯿﺒﻚ أو اﻟﺼﯿﺪﻟﻰ ﻗﺒﻞ ﺗﻨﺎول ھﺎﯾﻤﻮﻛﺲ:

·  إذا ﻛﺎن ﻟﺪﯾﻚ ﺣﻤﻰ ﻏﺪﯾﺔ )ﻋﺪوى ﻓﯿﺮوﺳﯿﺔ ﺗﺴﺒﺐ إﺣﺘﻘﺎن ﻓﻰ اﻟﺤﻠﻖ و إرﺗﻔﺎع ﻓﻰ درﺟﺔ اﻟﺤﺮارة و ﺗﻌﺐ و أﻟﻢ ﻓﻰ

اﻟﻌﻀﻼت و ﺻﺪاع(

·  إذا ﻛﻨﺖ ﺗﻌﺎﻧﻰ ﻣﻦ ﻣﺸﺎﻛﻞ ﻓﻰ اﻟﻜﺒﺪ أو اﻟﻜﻠﻰ

·  إذا ﺗﻨﺎوﻟﺖ ھﺎﯾﻤﻮﻛﺲ ﻟﻤﺪة طﻮﯾﻠﺔ ﻓﻘﺪ ﯾﺆدى ذﻟﻚ إﻟﻰ زﯾﺎدة ﻧﻤﻮ اﻟﺒﻜﺘﯿﺮﯾﺎ اﻟﺘﻰ ﺳﺘﺼﺒﺢ ﻣﻘﺎوﻣﺔ ﻟﻠﻤﻀﺎد اﻟﺤﯿﻮﯾﺔ

·  إذا اﺳﺘﺨﺪم ﻓﻰ اﻷطﻔﺎل اﻟﻤﻮﻟﻮدﯾﻦ ﻣﺒﻜﺮا أو ﺣﺪﯾﺜﻰ اﻟﻮﻻدة. ﯾﻨﺒﻐﻰ ﻣﺮاﻗﺒﺔ وظﺎﺋﻒ اﻟﻜﻠﻰ و اﻟﻜﺒﺪ و اﻟﺪم.

·  إذا ﻛﻨﺖ ﻻ ﺗﺘﺒﻮل ﺑﺼﻮرة ﻣﻨﺘﻈﻤﺔ

·  ﺗﻢ رﺻﺪ ﺣﺎﻻت ﻧﺎدرة ﻣﻦ إطﺎﻟﺔ ﻏﯿﺮ طﺒﯿﻌﯿﺔ ﻓﻰ وﻗﺖ اﻟﺒﺮوﺛﺮوﻣﺒﯿﻦ )زﯾﺎدة (INR ﻓﻰ اﻟﻤﺮﺿﻰ اﻟﺬﯾﻦ ﯾﺴﺘﺨﺪﻣﻮن

أﻣﻮﻛﺴﯿﺴﯿﻠﻠﯿﻦ و ﻣﻀﺎدات اﻟﺘﺠﻠﻂ. و ﻟﺬﻟﻚ ﯾﻨﺒﻐﻰ اﺳﺘﺨﺪام وﺳﺎﺋﻞ ﻣﻨﺎﺳﺒﺔ ﻟﻠﻤﺮاﻗﺒﺔ ﻋﻨﺪﻣﺎ ﯾﺘﻢ وﺻﻔﮭﻢ ﻣﻌﺎ. ﻗﺪ ﯾﻜﻮن

ﺗﻌﺪﯾﻞ ﺟﺮﻋﺔ ﻣﻀﺎد اﻟﺘﺠﻠﻂ ﺿﺮورﯾﺎ ﻟﻤﻮاﺻﻠﺔ اﻟﻤﺴﺘﻮى اﻟﻤﻄﻠﻮب ﻣﻦ ﻣﻔﻌﻮل ﻣﻀﺎد اﻟﺘﺠﻠﻂ.

   ﺗﻔﺎﻋﻼت ﺟﺎﻧﺒﯿﺔ ﺟﻠﺪﯾﺔ ﺷﺪﯾﺪة:

·  ﻗﺪ ﯾﺘﺴﺒﺐ اﻷﻣﻮﻛﺴﯿﺴﯿﻠﯿﻦ ﻓﻲ ﺣﺪوث ﺗﻔﺎﻋﻼت ﺟﺎﻧﺒﯿﺔ ﺟﻠﺪﯾﺔ ﺷﺪﯾﺪة، ﻣﺜﻞ ﻣﺘﻼزﻣﺔ ﺳﺘﯿﻔﻨﺰ ﺟﻮﻧﺴﻮن، وﺗﺤﻠﻞ اﻟﺒﺸﺮة اﻟﻨﺨﺮي اﻟﺴﻤﻲ، واﻟﺘﻔﺎﻋﻞ اﻟﺪواﺋﻲ ﻣﻊ زﯾﺎدة ﻛﺮات اﻟﺪم اﻟﺒﯿﻀﺎء واﻷﻋﺮاض اﻟﺠﺎﻧﺒﯿﺔ اﻟﻌﺎﻣﺔ ، واﻟﺒﺜﻮر اﻟﻄﻔﺤﯿﺔ اﻟﻌﺎﻣﺔ اﻟﺤﺎدة. إذا أﺻﯿﺐ اﻟﻤﺮﺿﻰ ﺑﻄﻔﺢ ﺟﻠﺪي ، ﯾﺠﺐ ﻣﺮاﻗﺒﺘﮭﻢ ﻋﻦ ﻛﺜﺐ وإﯾﻘﺎف اﻷﻣﻮﻛﺴﯿﺴﯿﻠﯿﻦ إذا ﺗﻄﻮرت

اﻻﻋﺮاض.

·  إذا ﻟﻢ ﺗﻜﻦ ﻣﺘﺄﻛﺪًا ﻣﻤﺎ إذا ﻛﺎن أي ﻣﻤﺎ ﺳﺒﻖ ﯾﻨﻄﺒﻖ ﻋﻠﯿﻚ ، ﻓﺘﺤﺪث إﻟﻰ طﺒﯿﺒﻚ أو اﻟﺼﯿﺪﻟﻲ.

ﺗﺤﺎﻟﯿﻞ اﻟﺪم واﻟﺒﻮل

طﺒﯿﺒﻚ أو اﻟﻤﻤﺮﺿﺔ أﻧﻚ ﺗﺘﻨﺎول ھﺎﯾﻤﻮﻛﺲ. ﻓﻘﺪ ﯾﺆﺛﺮ ھﺎﯾﻤﻮﻛﺲ ﻓﻰ ﻧﺘﺎﺋﺞ ھﺬه اﻟﺘﺤﺎﻟﯿﻞ.

ﺗﻨﺎول أدوﯾﺔ أﺧﺮى

ﯾﺮﺟﻰ إﺧﺒﺎر اﻟﻄﺒﯿﺐ أو اﻟﺼﯿﺪﻟﻲ إذا ﻛﻨﺖ ﺗﺘﻨﺎول أو ﺗﻨﺎوﻟﺖ ﻓﻲ اﻵوﻧﺔ اﻷﺧﯿﺮة أي أدوﯾﺔ أﺧﺮى، ﺑﻤﺎ ﻓﻲ ذﻟﻚ اﻷدوﯾﺔ اﻟﺘﻲ ﺗﻢ

اﻟﺤﺼﻮل ﻋﻠﯿﮭﺎ دون وﺻﻔﺔ طﺒﯿﺔ. ﻗﺪ ﯾﺆﺛﺮ ھﺎﯾﻤﻮﻛﺲ ﻓﻰ ﻋﻤﻞ ﺑﻌﺾ اﻷدوﯾﺔ ﻛﻤﺎ ﻗﺪ ﺗﺆﺛﺮ ﺑﻌﺾ اﻷدوﯾﺔ ﻓﻰ ﻋﻤﻞ ھﺎﯾﻤﻮﻛﺲ.

·  ﻓﻰ ﺑﻌﺾ اﻟﻤﻄﺒﻮﻋﺎت ھﻨﺎك ﺣﺎﻻت ﻧﺎدرة ﻣﻦ زﯾﺎدة (INR) ﻓﻰ اﻟﻤﺮﺿﻰ اﻟﺬﯾﻦ ﯾﺴﺘﺨﺪﻣﻮن اﺳﯿﻨﻮﻛﻮﻣﺎرول أو

وارﻓﺎرﯾﻦ ﻣﻊ أﻣﻮﻛﺴﯿﺴﯿﻠﻠﯿﻦ. ﻟﻮ ﻛﺎن ﺗﻨﺎوﻟﮭﻤﺎ ﻣﻌﺎ ﺿﺮورﯾﺎ ﯾﻨﺒﻐﻰ ﻣﺮاﻗﺒﺔ وﻗﺖ اﻟﺒﺮوﺛﺮوﻣﺒﯿﻦ أو INR ﺑﺤﺮص ﻋﻨﺪ إﺿﺎﻓﺔ أو ﺳﺤﺐ أﻣﻮﻛﺴﯿﺴﯿﻠﻠﯿﻦ )اﻧﻈﺮ ﻓﻘﺮة .2 ﻣﺎ ﯾﺠﺐ ﻣﺮاﻋﺎﺗﮫ ﻗﺒﻞ ﺗﻨﺎول ھﺎﯾﻤﻮﻛﺲ(

·  إذا ﻛﻨﺖ ﺗﺘﻨﺎول اﻟﻮﺑﯿﻮرﯾﻨﻮل )اﻟﻤﺴﺘﺨﺪم ﻟﻌﻼج اﻟﻨﻘﺮس( ﻣﻊ ھﺎﯾﻤﻮﻛﺲ ، ﻓﻤﻦ اﻟﻤﺤﺘﻤﻞ أن ﯾﻜﻮن ﻟﺪﯾﻚ رد ﻓﻌﻞ

ﺗﺤﺴﺴﻲ ﻓﻲ اﻟﺠﻠﺪ.

·  إذا ﻛﻨﺖ ﺗﺘﻨﺎول ﺑﺮوﺑﻨﯿﺴﯿﺪ )ﻟﻠﻮﻗﺎﯾﺔ ﻣﻦ اﻟﻨﻘﺮس.( ﻗﺪ ﯾﻘﺮر طﺒﯿﺒﻚ ﺗﻌﺪﯾﻞ ﺟﺮﻋﺘﻚ ﻣﻦ ھﺎﯾﻤﻮﻛﺲ.

·  إذا ﺗﻢ ﺗﻨﺎول أدوﯾﺔ ﻟﻠﻤﺴﺎﻋﺪة ﻓﻲ وﻗﻒ ﺗﺠﻠﻂ اﻟﺪم )ﻣﺜﻞ اﻟﻮارﻓﺎرﯾﻦ( ﻣﻊ ھﺎﯾﻤﻮﻛﺲ ، ﻓﻘﺪ ﺗﻜﻮن ھﻨﺎك ﺣﺎﺟﺔ

ﻟﻔﺤﺺ دم إﺿﺎﻓﻲ.

·  إذا ﻛﻨﺖ ﺗﺘﻨﺎوﻟﯿﻦ ﻣﻮاﻧﻊ اﻟﺤﻤﻞ اﻟﻔﻤﻮﯾﺔ ، ﻓﺴﻮف ﺗﺤﺘﺎﺟﯿﻦ إﻟﻰ اﺳﺘﺨﺪام اﺣﺘﯿﺎطﺎت إﺿﺎﻓﯿﺔ ﻟﻤﻨﻊ اﻟﺤﻤﻞ ، ﻣﺜﻞ

ﺗﺨﺘﻠﻒ اﻟﺠﺮﻋﺔ ﺣﺴﺐ ﻧﻮع اﻟﺠﺮاﺣﺔ. ﯾﻤﻜﻦ أﯾ  ًﻀﺎ إﻋﻄﺎء أدوﯾﺔ أﺧﺮى ﻓﻲ ﻧﻔﺲ اﻟﻮﻗﺖ

- ﯾﻤﻜﻦ ﻟﻄﺒﯿﺒﻚ أو اﻟﺼﯿﺪﻟﻲ أو اﻟﻤﻤﺮﺿﺔ ﺗﺰوﯾﺪك ﺑﻤﺰﯾﺪ ﻣﻦ اﻟﺘﻔﺎﺻﯿﻞ.

 

إذا ﻛﺎﻧﺖ ﻟﺪﯾﻚ ﻣﺸﺎﻛﻞ ﺑﺎﻟﻜﻠﻰ و اﻟﻜﺒﺪ :

·  إذا ﻛﻨﺖ ﺗﻌﺎﻧﻲ ﻣﻦ ﻣﺸﺎﻛﻞ ﺑﺎﻟﻜﻠﻰ ﻓﻘﺪ ﺗﺤﺘﺎج إﻟﻰ ﺗﻘﻠﯿﻞ اﻟﺠﺮﻋﺔ . ﻗﺪ ﯾﺨﺘﺎر ﻟﻚ طﺒﯿﺒﻚ ﺗﺮﻛﯿﺰ ﻣﺨﺘﻠﻒ أو دواء ﻣﺨﺘﻠﻒ.

·  إذا ﻛﻨﺖ ﺗﻌﺎﻧﻲ ﻣﻦ ﻣﺸﺎﻛﻞ ﺑﺎﻟﻜﺒﺪ ﻓﻘﺪ ﺗﺤﺘﺎج إﻟﻰ ﻋﻤﻞ ﺗﺤﺎﻟﯿﻞ دم ﺑﺸﻜﻞ ﻣﻜﺜﻒ ﻟﺘﺤﺪﯾﺪ ﻛﯿﻔﯿﺔ ﻋﻤﻞ اﻟﻜﺒﺪ.

إذا ﺗﻨﺎوﻟﺖ اﻛﺜﺮ ﻣﻤﺎ ﯾﺠﺐ:

إذا اﺑﺘﻌﻠﺖ أو )ﺷﺨﺺ آﺧﺮ( اﻟﻜﺜﯿﺮ ﻣﻦ ھﺎﯾﻤﻮﻛﺲ ﻓﻲ ﻧﻔﺲ اﻟﻮﻗﺖ , أو اﻋﺘﻘﺪت ﺑﺄن طﻔﻼ ﻗﺪ اﺑﺘﻠﻊ ﺑﻌﻀﺎ ﻣﻦ ﻛﺒﺴﻮﻻت ھﺎﯾﻤﻮﻛﺲ,

اﺗﺼﻞ ﺑﻘﺴﻢ اﻟﻄﻮارئ ﻓﻲ أﻗﺮب ﻣﺴﺘﺸﻔﻰ أو اﺧﺒﺮ طﺒﯿﺒﻚ. ﻗﺪ ﺗﻜﻮن اﻷﻋﺮاض ھﻰ اﺿﻄﺮاب ﻓﻰ اﻟﻤﻌﺪة )اﻟﺸﻌﻮرﺑﺎﻹﻋﯿﺎء أو اﻹﯾﺎء أو اﻹﺳﮭﺎل( أو وﺟﻮد ﺑﻠﻮرات ﻓﻰ اﻟﺒﻮل ﻣﻤﺎ ﯾﻈﮭﺮه

ﻣﻌﻜﺮ أو وﺟﻮد ﻣﺸﺎﻛﻞ ﻓﻰ اﻟﺘﺒﻮل. ﺧﺬ اﻟﺪواء ﻟﺘﻌﺮﺿﮫ ﻟﻠﻄﺒﯿﺐ.

 

إذا ﻧﺴﯿﺖ ﺗﻨﺎول اﻟﺠﺮﻋﺔ:

إذا ﻧﺴﯿﺖ اﺧﺬ اﻟﺠﺮﻋﺔ, ﻗﻢ ﺑﺄﺧﺬھﺎ ﻓﻮر ﺗﺬﻛﺮك , اﻧﺘﻈﺮ ﻟﻤﺪة أرﺑﻊ ﺳﺎﻋﺎت ﻗﺒﻞ ﺗﻨﺎول اﻟﺠﺮﻋﺔ اﻟﺘﺎﻟﯿﺔ. ﻻ ﺗﺘﻨﺎول ﺟﺮﻋﺔ ﻣﺰدوﺟﺔ

ﻟﺘﻌﻮض اﻟﺠﺮﻋﺔ اﻟﻤﻨﺴﯿﺔ.

إذا ﺗﻮﻗﻔﺖ ﻋﻦ ﺗﻨﺎول ھﺎﯾﻤﻮﻛﺲ:

داوم ﻋﻠﻰ ﺗﻨﺎول ھﺎﯾﻤﻮﻛﺲ ﺣﺘﻰ ﻧﮭﺎﯾﺔ اﻟﻌﻼج ﺣﺘﻰ ﻟﻮ ﺷﻌﺮت ﺑﺘﺤﺴﻦ. ﻷﻧﻚ ﺗﺤﺘﺎج ﻛﻞ ﺟﺮﻋﺔ ﻟﻠﻤﺴﺎﻋﺪة ﻓﻲ اﻟﻘﻀﺎء ﻋﻠﻰ اﻟﻌﺪوى.

ﻷن ﺑﻌﺾ اﻟﺒﻜﺘﯿﺮﯾﺎ ﻗﺪ ﺗﺴﺒﺐ ﻋﻮدة اﻟﻌﺪوى ﻣﺮة أﺧﺮى. ﯾﺠﺐ أن ﯾﺴﺘﻤﺮ اﻟﻌﻼج ﯾﻮﻣﯿﻦ إﻟﻰ ﺛﻼﺛﺔ أﯾﺎم ﺑﻌﺪ إﺧﺘﻔﺎء اﻷﻋﺮاض.

ﻻ ﺗﺘﻨﺎول ھﺎﯾﻤﻮﻛﺲ ﻷﻛﺜﺮ ﻣﻦ أﺳﺒﻮﻋﯿﻦ. إذا اﺳﺘﻤﺮﯾﺖ ﺑﺎﻟﺸﻌﻮر أﻧﻚ ﺑﺤﺎﻟﺔ ﻏﯿﺮ ﺟﯿﺪة , اذھﺐ ﻟﺮؤﯾﺔ اﻟﻄﺒﯿﺐ. ﻣﺮض اﻟﻘﻼع )ﻋﺪوى اﻟﺨﻤﯿﺮة ﻣﻦ اﻟﻤﻨﺎطﻖ اﻟﺮطﺒﺔ ﻣﻦ اﻟﺠﺴﻢ( ﯾﻤﻜﻦ أن ﺗﺘﻄﻮر إذا ﺗﻢ ﺗﻨﺎول ھﺎﯾﻤﻮﻛﺲ ﻟﻔﺘﺮة طﻮﯾﻠﺔ. إذا ﺣﺪث

ھﺬا، وﻛﻨﺖ ﻗﺪ ﺗﻨﺎوﻟﺖ ھﺎﯾﻤﻮﻛﺲ ﻟﻔﺘﺮة أطﻮل ﻣﻦ اﻟﻤﻮﺻﻰ ﺑﮭﺎ، أﺧﺒﺮ طﺒﯿﺒﻚ.

إذا ﻛﺎن ﻟﺪﯾﻚ أى أﺳﺌﻠﺔ إﺿﺎﻓﯿﺔ ﻋﻦ اﺳﺘﺨﺪام ھﺬا اﻟﺪواء. اﺳﺄل طﺒﯿﺒﻚ أو اﻟﺼﯿﺪﻟﻰ.

ﻣﺜﻞ ﺟﻤﯿﻊ اﻷدوﯾﺔ, ھﺎﯾﻤﻮﻛﺲ ﻗﺪ ﺗﺴﺒﺐ أﻋﺮاض ﺟﺎﻧﺒﯿﺔ ، وإن ﻟﻢ ﺗﻜﻦ ﺗﺤﺪث ﻟﻜﻞ ﻣﻦ ﯾﺘﻨﺎول ھﺬا اﻟﺪواء. إذا ﻻﺣﻈﺖ أن أﯾﺎ ﻣﻦ ھﺬه اﻷﻋﺮاض اﻟﺠﺎﻧﺒﯿﺔ أﺻﺒﺢ ﺟﺴﯿﻤﺎ، أو إذا ﻻﺣﻈﺖ ظﮭﻮر أى أﻋﺮاض ﺟﺎﻧﺒﯿﺔ ﻟﻢ ﺗﺮد ﻓﻰ ھﺬه اﻟﻨﺸﺮة

ﻓﺈﻧﮫ ﯾﺮﺟﻰ أن ﺗﺨﺒﺮ طﺒﯿﺒﻚ اﻟﻤﻌﺎﻟﺞ أو اﻟﺼﯿﺪﻟﻰ اﻟﺬى ﺗﺘﻌﺎﻣﻞ ﻣﻌﮫ ﺑﺸﺄﻧﮭﺎ

ﺗﻮﻗﻒ ﻋﻦ ﺗﻨﺎول ھﺎﯾﻤﻮﻛﺲ و اﺗﺼﻞ ﺑﻄﺒﯿﺒﻚ ﻋﻨﺪﻣﺎ ﺗﺸﻌﺮ ﺑﺄي ﻣﻦ اﻷﻋﺮاض اﻟﺘﺎﻟﯿﺔ :

اﻷﻋﺮاض اﻟﺘﺎﻟﯿﺔ ﻧﺎدرة ﺟﺪا )ﺗﺆﺛﺮ ﻓﻰ أﻗﻞ ﻣﻦ 1 ﻓﻰ ﻛﻞ 10.000 ﺷﺨﺺ(

·  ﻓﺮط اﻟﺤﺴﺎﺳﯿﺔ أو اﻟﺤﺴﺎﺳﯿﺔ اﻟﺸﺪﯾﺪة ﺑﻤﺎ ﻓﻲ ذﻟﻚ اﻟﻄﻔﺢ ، ﺣﻜﺔ، اﻟﺘﮭﺎب اﻟﻔﻢ أو اﻟﻌﯿﻨﯿﻦ، وﺗﻮرم ﻓﻲ اﻟﻮﺟﮫ واﻟﺸﻔﺘﯿﻦ

واﻟﻠﺴﺎن أو اﻟﺤﻠﻖ أو ﻣﺸﺎﻛﻞ ﻓﻲ اﻟﺘﻨﻔﺲ.

·  ﺑﻘﻊ ﺣﻤﺮاء ﻣﺴﻄﺤﺔ ﺗﺤﺖ ﺳﻄﺢ اﻟﺠﻠﺪ أو ﻛﺪﻣﺎت ﻓﻲ اﻟﺠﻠﺪ. ھﺬا ھﻮ ﻧﺘﯿﺠﺔ رد اﻟﻔﻌﻞ اﻟﺘﺤﺴﺴﻲ اﻟﻨﺎﺟﻢ ﻋﻦ اﻟﺘﮭﺎب ﺟﺪران

·  ﯾﻤﻜﻦ أن ﺗﺤﺪث ردة ﻓﻌﻞ ﺗﺤﺴﺴﯿﺔ ﻣﺘﺄﺧﺮة ﺑﻌﺪ 7 إﻟﻰ 12 ﯾﻮم ﻣﻦ ﺗﻨﺎول ھﺎﯾﻤﻮﻛﺲ , ﺑﻌﺾ اﻷﻋﺮاض ﺗﺘﻀﻤﻦ , طﻔﺢ

ﺟﻠﺪي, ﺣﻤﻰ, اﻻم ﻓﻲ اﻟﻤﻔﺎﺻﻞ و ﺗﻀﺨﻢ اﻟﻐﺪد اﻟﻠﯿﻤﻔﺎوﯾﺔ ﺧﺎﺻﺔ ﻣﻨﻄﻘﺔ اﻹﺑﻂ.

·  ﺗﻔﺎﻋﻞ ﺟﻠﺪى ﯾﺴﻤﻰ )ﺣﻤﺎﻣﻰ ﻋﺪﯾﺪة اﻷﺷﻜﺎل( وﺗﺸﻤﻞ أﻋﺮاﺿﮫ: ظﮭﻮر ﺑﻘﻊ ﺣﻤﺮاء وردﯾﺔ ﻋﻠﻰ اﻟﺠﻠﺪ ﺗﺜﯿﺮ اﻟﺤﻜﺔ و

ﻣﻨﺎطﻖ ﻣﺮﺗﻔﻌﺔ ﻋﻠﻰ اﻟﺠﻠﺪ ﺗﺸﺒﮫ ﺧﻼﯾﺎ اﻟﻨﺤﻞ و ﻣﻨﺎطﻖ ﻟﯿﻨﺔ ﻋﻠﻰ اﻟﻔﻢ و اﻟﻌﯿﻦ و اﻟﻤﻨﺎطﻖ اﻟﺤﺴﺎﺳﺔ.

·  ﺗﻔﺎﻋﻼت ﺟﻠﺪﯾﺔ ﺧﻄﯿﺮة اﺧﺮى ﻣﺜﻞ ﺗﻐﯿﺮ ﻓﻲ ﻟﻮن اﻟﺠﻠﺪ, اﻹﻧﺘﻔﺎﺧﺎت ﺗﺤﺖ اﻟﺠﻠﺪ, ﺗﻘﺮﺣﺎت, اﺣﻤﺮار, أﻟﻢ , ﺗﻘﺸﺮ.

·  درﺟﺔ ﺣﺮارة ﻋﺎﻟﯿﺔ )ﺣﻤﻰ( , ﻗﺸﻌﺮﯾﺮة, اﻟﺘﮭﺎب اﻟﺤﻠﻖ أو أﻋﺮاض اﺧﺮى ﻟﻠﻌﺪوى , أو ﺗﻌﺮﺿﻚ ﻟﻠﻜﺪﻣﺎت ﺑﺴﮭﻮﻟﺔ.

·  أﻋﺮاض ﺟﺎﻧﺒﯿﺔ ﺷﺪﯾﺪة ﯾﻤﻜﻦ أن ﺗﺤﺪث ﻟﻠﻜﺒﺪ و ھﻲ ﻗﺎﺑﻠﺔ ﻟﻠﻌﻮدة. ارﺗﺒﻂ ﺑﺸﻜﻞ أﺳﺎﺳﻲ ﺑﺎﻷﺷﺨﺎص اﻟﺬﯾﻦ ﻋﻮﻟﺠﻮا ﻟﻔﺘﺮة

طﻮﯾﻠﺔ ، ﻣﻦ اﻟﺬﻛﻮر وﻛﺒﺎر اﻟﺴﻦ. ﯾﺠﺐ أن ﺗﺨﺒﺮ طﺒﯿﺒﻚ ﻋﻠﻰ وﺟﮫ اﻟﺴﺮﻋﺔ إذا ﺣﺼﻠﺖ ﻋﻠﻰ:

o إﺳﮭﺎل ﺷﺪﯾﺪ ﻣﻊ ﻧﺰﯾﻒ o  ظﮭﻮر ﺑﺜﻮر أو اﺣﻤﺮار أو ﻛﺪﻣﺎت ﻓﻲ اﻟﺠﻠﺪ

o  اﺻﻔﺮار اﻟﺠﻠﺪ أو ﺑﯿﺎض اﻟﻌﯿﻨﯿﻦ )اﻟﯿﺮﻗﺎن.( اﻧﻈﺮ أﯾ  ًﻀﺎ ﻓﻘﺮ اﻟﺪم أدﻧﺎه واﻟﺬي ﻗﺪ ﯾﺆدي إﻟﻰ اﻟﯿﺮﻗﺎن.

ﯾﻤﻜﻦ أن ﯾﺤﺪث ھﺬا ﺣﺘﻰ ﻋﺪة أﺳﺎﺑﯿﻊ ﺑﻌﺪ اﻟﻌﻼج.

.ﻓﻲ ﺣﺎﻟﺔ ﺣﺪوث أي ﻣﻤﺎ ﺳﺒﻖ ، ﺗﻮﻗﻒ ﻋﻦ ﺗﻨﺎول اﻟﺪواء واﺳﺘﺸﺮ طﺒﯿﺒﻚ ﻋﻠﻰ اﻟﻔﻮر

ﻓﻲ ﺑﻌﺾ اﻷﺣﯿﺎن ﻗﺪ ﺗﺼﺎب ﺑﺮدود ﻓﻌﻞ ﺟﻠﺪﯾﺔ أﻗﻞ ﺣﺪة ﻣﺜﻞ: طﻔﺢ ﺟﻠﺪي ﺧﻔﯿﻒ ﯾﺴﺒﺐ اﻟﺤﻜﺔ )ﺑﻘﻊ داﺋﺮﯾﺔ ﺣﻤﺮاء وردﯾﺔ اﻟﻠﻮن.( ﻣﻨﺎطﻖ ﻣﻨﺘﻔﺨﺔ "ﺗﺸﺒﮫ ﺧﻼﯾﺎ اﻟﻨﺤﻞ" ﻋﻠﻰ اﻻذرع أو .اﻟﺴﺎﻗﯿﻦ أو اﻟﺮاﺣﺘﯿﻦ أو اﻟﯿﺪﯾﻦ أو اﻟﻘﺪﻣﯿﻦ. ھﺬا ﻏﯿﺮ ﺷﺎﺋﻊ )ﯾﺼﯿﺐ أﻗﻞ ﻣﻦ 1 ﻓﻲ 100 ﺷﺨﺺ(

·  ﻻ ﺗﺘﻨﺎول ھﺎﯾﻤﻮﻛﺲ ﺑﻌﺪ اﻧﺘﮭﺎء ﺗﺎرﯾﺦ ﺻﻼﺣﯿﺘﮫ اﻟﻤﻮﺿﺢ ﻋﻠﻰ اﻟﻌﺒﻮة . ﯾﺸﯿﺮ ﺗﺎرﯾﺦ اﻹﻧﺘﮭﺎء إﻟﻰ آﺧﺮ ﯾﻮم ﻓﻲ اﻟﺸﮭﺮ.

Hymox

Capsules: Each capsule contains 250 mg or 500 mg amoxicillin as the trihydrate B.P. Tablets: Each film coated tablet contains 875 mg amoxicillin as the trihydrate B.P.

Hymox Capsules and film coated tablets.

Treatment of Infection: Hymox is a broad spectrum antibiotic indicated for the treatment of commonly occurring bacterial infections such as:

 

Upper respiratory tract infections Otitis media

Acute and chronic bronchitis Chronic bronchial sepsis Lobar and bronchopneumonia

Cystitis, urethritis, pyelonephritis Bacteriuria in pregnancy

Gynaecological infections including puerperal sepsis and septic abortion Gonorrhoea

Peritonitis

 

Intra-abdominal sepsis Septicaemia

Bacterial endocarditis

 

Typhoid and paratyphoid fever Skin and soft tissue infections

Dental abscess (as an adjunct to surgical management)

 

Helicobacter pylori eradication in peptic (duodenal and gastric) ulcer disease.

 

In children with urinary tract infection the need for investigation should be considered.

 

Prophylaxis of endocarditis: Hymox may be used for the prevention of bacteraemia, associated with procedures such as dental extraction, in patients at risk of developing bacterial endocarditis.

 

Consideration should be given to official local guidance (e.g. national requirements) on the appropriate use of antibacterial agents.”Susceptibility of the causative organism to the treatment should be tested (if possible), although the therapy may be initiated before the results are available.

 


Treatment of Infection:

 

Adult dosage (including elderly patients):

 

Standard adult dosage: 250 mg three times daily, increasing to 500 mg three times daily for more severe infections.

 

High dosage therapy (maximum recommended oral dosage 6 g daily in divided doses): A dosage of 3 g twice daily is recommended in appropriate cases for the treatment of severe or recurrent purulent infection of the respiratory tract.

 

Short course therapy: Simple acute urinary tract infection: two 3 g doses with 10-12 hours between the doses. Dental abscess: two 3 g doses with 8 hours between the doses.

Gonorrhoea: single 3 g dose.

 

Dosage in impaired renal function:

 

The dose should be reduced in patients with severe renal function impairment. In patients with a creatinine clearance of less than 30 ml/min an increase in the dosage interval and a reduction in the total daily dose is recommended (see section 4.4 and 5.2):

Hymox is recommended at a dose of twice daily in association with a proton pump inhibitor and antimicrobial agents as detailed below:

 

Omeprazole 40 mg daily, Amoxicillin 1G BID, Clarithromycin 500mg

BID x 7days or

Omeprazole 40mg daily, Amoxicillin750mg-1G BID, Metronidazole 400mg TID x 7 days

Children weighing < 40 kg

 

The daily dosage for children is 40 - 90 mg/kg/day in two to three divided doses* (not exceeding 3 g/day) depending on the indication, severity of the disease and the susceptibility of the pathogen (see special dosage recommendations below and sections 4.4, 5.1 and 5.2).

 

* PK/PD data indicate that dosing three times daily is associated with enhanced efficacy, thus twice daily dosing is only recommended when the dose is in the upper range.

 

Children weighing more than 40 kg should be given the usual adult dosage.

Hymox Paediatric Suspension is recommended for children under six months of age.

 

Special dosage recommendation

 

Tonsillitis: 50 mg/kg/day in two divided doses.

 

Acute otitis media: In areas with high prevalence of pneumococci with reduced susceptibility to penicillins, dosage regimens should be guided by national/local recommendations. In severe or recurrent acute otitis media, especially where compliance may be a problem, 750 mg twice a day for two days may be used as an alternative course of treatment in children aged 3 to 10 years.

 

Early Lyme disease (isolated erythema migrans): 50 mg/kg/day in three divided doses, over 14-21days.

Prophylaxis of endocarditis: see table on next page.

 

Administration: Oral:

 

Treatment should be continued for 2 to 3 days following the disappearance of symptoms. It is recommended that at least 10 days treatment be given for any infection caused by beta-haemolytic streptococci in order to achieve eradication of the organism.

Note 1. If prophylaxis with 'Hymox' is given twice within one month, emergence of resistant streptococci is unlikely to be a problem.

Alternative antibiotics are recommended if more frequent prophylaxis is required, or if the patient has received a course of treatment with a penicillin during the previous month.

Patient having general anaesthetic: if oral antibiotics considered to be appropriate.

Initially 3 g 'Hymox' orally 4 hours prior to anaesthesia, followed by 3 g orally (or 1 g IV or IM if oral dose not tolerated) as soon as possible after the operation.

 

Patient having general anaesthetic: if oral antibiotics not appropriate.

1 g 'Hymox' IV or IM immediately before induction; with 500 mg orally, 6 hours later.

 

Note 2

 

To minimise pain on injection, 'Hymox' may be given as two injections of 500 mg dissolved in sterile 1%

 

 

 

 

 

lidocaime solution (see Administration).

Dental procedures: patients for whom referral to hospital is recommended:

 

a)  Patients to be given a general anaesthetic who have been given a penicillin in the previous month.

 

b)  Patients to be given a general anaesthetic who have a prosthetic heart valve.

 

c)  Patients who have had one or more attacks of endocarditis.

Initially: 1 g 'Hymox' IV or IM with 120 mg gentamicin IV or IM immediately prior to anaesthesia (if given) or 15 minutes prior to dental procedure.

 

Followed by (6

hours later): 500 mg 'Hymox' orally.

50 mg amoxicillin/kg body weight given as a single dose one hour preceding the surgical procedure

Note 3. 'Hymox' and gentamicin should not be mixed in the same syringe.

Genitourinary Surgery or Instrumentation: prophylaxis for patients who have no urinary tract infection and who are to have genito-urinary surgery or instrumentation under general anaesthesia.

Initially: 1 g 'Hymox' IV or IM with 120 mg gentamicin IV or IM, immediately before

induction.

In the case of Obstetric and Gynaecological Procedures and Gastrointestinal Procedures– routine prophylaxis is recommended only for patients with prosthetic heart valves.

Hymox is a penicillin and should not be given to penicillin-hypersensitive patients. Attention should be paid to possible cross-sensitivity with other beta-lactam antibiotics eg. cephalosporins.

Before initiating therapy with amoxicillin, careful enquiry should be made concerning previous hypersensitivity reactions to penicillins, cephalosporins.

 

Serious and occasionally fatal hypersensitivity (anaphylactoid) reactions have been reported in patients on penicillin therapy. These reactions are more likely to occur in individuals with a history of hypersensitivity to beta-lactam antibiotics (see 4.3).

 

Erythematous (morbilliform) rashes have been associated with glandular fever in patients receiving amoxicillin.

 

Prolonged use may also occasionally result in overgrowth of non-susceptible organisms.

 

In patients with reduced urine output, crystalluria has been observed very rarely, predominantly with parenteral therapy. During the administration of high doses of amoxicillin, it is advisable to maintain adequate fluid intake and urinary output in order to reduce the possibility of amoxicillin crystalluria (see Section 4.9 Overdose).

 

In patients with renal impairment, the rate of excretion of amoxicillin will be reduced depending on the degree of impairment and it may be necessary to reduce the total daily unit amoxicillin dosage accordingly (see section 4.2).

 

Abnormal prolongation of prothrombin time (increased INR) has been reported rarely in patients receiving amoxicillin and oral anticoagulants. Appropriate monitoring should be undertaken when anticoagulants are prescribed concomitantly. Adjustments in the dose of oral anticoagulants may be necessary to maintain the desired level of anticoagulation (see sections 4.5 and 4.8)

Precaution should be taken in premature children and during the neonatal period: renal, hepatic and haematological functions should be monitored

Severe Cutaneous Adverse Reactions

 

Amoxicillin may cause severe cutaneous adverse reactions (SCAR), such as Stevens- Johnson syndrome (SJS), toxic epidermal necrolysis (TEN), drug reaction with eosinophilia and systemic symptoms (DRESS), and acute generalized exanthematous pustulosis (AGEP). If patients develop a skin rash they should be monitored closely and amoxicillin discontinued if lesions progress.


Probenecid decreases the renal tubular secretion of amoxicillin. Concurrent use with amoxicillin may result in increased and prolonged blood levels of amoxicillin.

 

In common with other antibiotics, amoxicillin may affect the gut flora, leading to lower oestrogen reabsorption and reduced efficacy of combined oral contraceptives.

 

Concurrent administration of allopurinol during treatment with amoxicillin can increase the likelihood of allergic skin reactions.

 

In the literature there are rare cases of increased international normalised ratio in patients maintained on acenocoumarol or warfarin and prescribed a course of amoxicillin. If co- administration is necessary, the prothrombin time or international normalised ratio should be carefully monitored with the addition or withdrawal of amoxicillin (see sections 4.4 and 4.8).

 

It is recommended that when testing for the presence of glucose in urine during amoxicillin treatment, enzymatic glucose oxidase methods should be used. Due to the high urinary concentrations of amoxicillin, false positive readings are common with chemical methods


 

Animal studies with Hymox have shown no teratogenic effects. The product has been in extensive clinical use since 1972 and its suitability in human pregnancy has been well documented in clinical studies. When antibiotic therapy is required during pregnancy, Hymox may be considered appropriate when the potential benefits outweigh the potential risks associated with treatment.

 

Use in lactation:

 

Amoxicillin may be given during lactation. With the exception of the risk of sensitisation associated with the excretion of trace quantities of amoxicillin in breast milk, there are no known detrimental effects for the breast-fed infant.


Adverse effects on the ability to drive or operate machinery have not been observed.


The following convention has been utilised for the classification of undesirable effects:-

Very common (>1/10), common (>1/100, <1/10), uncommon (>1/1000,<1/100), rare (>1/10,000, <1/1000), very rare (<1/10,000)

 

The majority of side effects listed below are not unique to amoxicillin and may occur when using other penicillins.

 

Unless otherwise stated, the frequency of adverse events has been derived from more than 30 years of post-marketing reports.

Infections and infestations

Very Rare:          Mucocutaneous candidiasis

Blood and lymphatic system disorders

Very rare:           Reversible leucopenia (including severe neutropenia or agranulocytosis), reversible thrombocytopenia and haemolytic anaemia.

 

Prolongation of bleeding time and prothrombin time (see section 4.4 – Special Warnings and Precautions for Use.

Immune system disorders

Very rare:           As with other antibiotics, severe allergic reactions, including angioneurotic oedema, anaphylaxis (see Section 4.4 - Special Warnings and Precautions for Use), serum sickness and hypersensitivity vasculitis.

If a hypersensitivity reaction is reported, the treatment must be discontinued. (See also Skin and subcutaneous tissue disorders).

Nervous system disorders

Very rare:           Hyperkinesia, dizziness and convulsions. Convulsions may occur in patients with impaired renal function or in those receiving high doses.

Gastrointestinal disorders

*Uncommon: Vomiting. Post-marketing Data

Very rare:           Antibiotic associated colitis (including pseudomembraneous colitis and haemorrhagic colitis).

Black hairy tongue

Superficial tooth discolouration has been reported in children. Good oral hygiene may help to prevent tooth discolouration as it can usually be removed by brushing.

 

Hepato-biliary disorders

Very rare:           Hepatitis and cholestatic jaundice. A moderate rise in AST and/or ALT.

The significance of a rise in AST and/or ALT is unclear.

Skin and subcutaneous tissue disorders

*Common:           Skin rash

*Uncommon:                             Urticaria and pruritus Post-marketing Data

Very rare:           Skin reactions such as erythema multiforme, Stevens-Johnson syndrome, toxic epidermal necrolysis, bullous and exfoliative dermatitis and acute generalised exanthematous pustulosis (AGEP), and drug reaction with eosinophilia and systemic symptoms (DRESS).

Renal and urinary tract disorders Very rare:                             Interstitial nephritis.

Very rare:             Crystalluria (see Section 4.9 Overdose)

*The incidence of these AEs was derived from clinical studies involving a total of approximately 6,000 adult and paediatric patients taking amoxicillin.

 
  

 


Gastrointestinal effects such as nausea, vomiting and diarrhoea may be evident and should be treated symptomatically with attention to the water/electrolyte balance. Amoxicillin crystalluria, in some cases leading to renal failure, has been observed (see Section 4.4 Special warnings and special precautions for use).

 

Amoxicillin may be removed from the circulation by haemodialysis.


Hymox is a broad spectrum antibiotic.

It is rapidly bactericidal and possesses the safety profile of a penicillin.

Hymox is well absorbed by the oral and parenteral routes. Oral administration, usually at convenient t.d.s. dosage, produces high serum levels independent of the time at which food is taken. Hymox gives good penetration into bronchial secretions and high urinary concentrations of unchanged antibiotic.

 

In preterm infants with gestational age 26-33 weeks, the total body clearance after intravenous dosing of amoxicillin, day 3 of life, ranged between 0.75 – 2 ml/min, very similar to the inuline clearance (GFR) in this population. Following oral administration,


the absorption pattern and the bioavailability of amoxicillin in small children may be different to that of adults. Consequently, due to the decreased CL, the exposure is expected to be elevated in this group of patients, although this increase in exposure may in part be diminished by decreased bioavailability when given orally.
