Cosyrel contains two active ingredients, bisoprolol fumarate and perindopril arginine in one tablet:

 

·        Bisoprolol  fumarate belongs to a group of medicine called beta-blockers. Beta-blockers slow down the heart rate and make the heart more efficient at pumping blood around the body.

·        Perindopril arginine is an angiotensin converting enzyme (ACE) inhibitor. It works by widening the blood vessels, which makes it easier for your heart to pump blood through them.

 

Cosyrel is used to treat high blood pressure (hypertension) and/or to reduce the risk of cardiac events, such as heart attack, in patients with stable coronary artery disease (a condition where the blood supply to the heart is reduced or blocked) and who have already had a heart attack and/or an operation to improve the blood supply to the heart by widening the vessels that supply it.

 

Instead of taking bisoprolol fumarate and perindopril arginine as separate tablets, you will take only one tablet of Cosyrel which contains both active ingredients in the same strength.


Do not take Cosyrel if you:

·       are allergic to bisoprol or any other beta-blocker, to perindopril or any other ACE inhibitor, or to any of the other ingredients of this medicine (listed in section 6),

·       have heart failure that suddenly becomes worse and/or that may require hospital treatment,

·       have a cardiogenic shock (a serious heart condition caused by very low blood pressure),

·       have a heart disease characterized by a slow or irregular heart rate (atrioventricular block second or third degree, sinoatrial block, sick sinus syndrome),

·       have a slow heart rate,

·       have very low blood pressure,

·       have severe asthma or severe chronic lung disease,

·       have severe blood circulation problems in your limbs (such as Raynaud’s syndrome), which may cause your fingers and toes to tingle or turn pale or blue,

·       have an untreated phaeochromocytoma, which is a rare tumour of the adrenal gland (medulla),

·       have metabolic acidosis, a condition where your blood contains too much acid,

·       have experienced symptoms such as wheezing, swelling of the face, tongue or throat, intense itching or severe skin rashes with previous ACE inhibitor treatment or if you or a member of your family have had these symptoms in any other circumstances (a condition called angioedema),

·       are more than 3 months pregnant (it is also better to avoid Cosyrel in early pregnancy - see pregnancy section),

·       have diabetes or impaired kidney function and you are treated with a blood pressure lowering medicine containing aliskiren,

·       are having dialysis or any other type of blood filtration. Depending on the machine that is used, Cosyrel may not be suitable for you,

·       have kidney problems where the blood supply to your kidneys is reduced (renal artery stenosis),

·       if you have taken or are currently taking sacubitril/valsartan, a medicine for heart failure, as the risk of angioedema (rapid swelling under the skin in an area such as the throat) is increased (see “Warning and Precaution” and “Other medicines and Cosyrel”).

 

Warnings and precautions

Talk to your doctor or pharmacist before taking Cosyrel, if you:

·       have diabetes,

·       have kidney problems (including kidney transplantation) or if you are receiving dialysis,

·       have a liver problem,

·       have aortic and mitral stenosis (narrowing of the main blood vessel leading from the heart) or hypertrophic cardiomyopathy (heart muscle disease) or renal artery stenosis (narrowing of the artery supplying the kidney with blood),

·       have abnormally increased levels of a hormone called aldosterone in your blood (primary aldosteronism),

·       have heart failure or any other heart problems such as minor disturbances in heart rhythm or severe chest pain at rest (Prinzmetal’s angina),

·       suffer from a collagen vascular disease (disease of the connective tissue) such as systemic lupus erythematosus or scleroderma,

·       are on a salt restricted diet or use salt substitutes which contain potassium (too much potassium in the blood can cause changes in the heart rates),

·       have recently suffered from diarrhea or vomiting, or are dehydrated (Cosyrel may cause a fall in blood pressure),

·       are to undergo LDL apheresis (which is removal of cholesterol from your blood by a machine),

·       have current antiallergic treatment or are going to have desensitisation treatment to reduce the effects of an allergy to bee or wasp stings,

·       are under strict fasting or diet,

·       are to undergo anaesthesia and/or major surgery,

·       have any problems with the circulation in your limbs,

·       have asthma or chronic lung disease,

·       have (or have had) psoriasis,

·       have a tumour of the adrenal gland (phaeochromocytoma),

·       have thyroid disorders (Cosyrel can hide symptoms of an overactive thyroid),

·       have angioedema (severe allergic reaction with swelling of the face, lips, tongue or throat with difficulty in swallowing or breathing). This may occur at any time during treatment. If you develop such symptoms, you should stop taking Cosyrel and see a doctor immediately,

·       are of black origin since you may have a higher risk of angioedema and this medicine may be less effective in lowering your blood pressure than in non-black patients,

·       are taking any of the following medicines used to treat high blood pressure:

-      an “angiotensin II receptor blocker” (ARBs) (also known as sartans - for example valsartan, telmisartan, irbesartan), in particular if you have diabetes-related kidney problems.

-      aliskiren.

Your doctor may check your kidney function, blood pressure, and the amount of electrolytes (e.g. potassium) in your blood at regular intervals. See also information under the heading “Do not take Cosyrel”.

·       are taking any of the following medicines, the risk of angioedema is increased:

-      racecadotril (used to treat diarrhea),

-      sirolimus, everolimus, temsirolimus and other drugs belonging to the class of so-called mTor inhibitors (used to avoid rejection of transplanted organs and for cancer).

-   sacubitril (available as fixed-dose combination with valsartan), used to treat long-term heart failure.

-    linagliptin, saxagliptin, sitagliptin, vildagliptin and other drugs belonging to the class of the also called gliptins (used to treat diabetes).

 

 

Do not suddenly stop taking Cosyrel since this can cause severe worsening of heart condition. Treatment should not be discontinued abruptly, especially in patients with coronary artery disease.

You must tell your doctor if you think you are (or might become) pregnant. Cosyrel is not recommended in early pregnancy, and must not be taken if you are more than 3 months pregnant, as it may cause serious harm to your baby if used at that stage (see pregnancy section).

 

Children and adolescents

Cosyrel is not recommended for use in children and adolescents younger than 18.

 

Other medicines and Cosyrel

Please tell your doctor or pharmacist if you are taking, have recently taken or might take any other medicines.

 

There are some medicines that may change the effect of Cosyrel or their effect may be changed by Cosyrel. This type of interaction could make one or both of the medicines less effective. Alternatively it could increase the risk or severity of side-effects.

 

Make sure to tell your doctor if you are taking any of the following medicines:

·       medicines used to control blood pressure or medicines for heart problems (such as amiodarone, amlodipine, clonidine, digitalis glycosides, diltiazem, disopyramide, felodipine, flecainide, lidocaine, methyldopa, moxonidine, procainamide, propafenone, quinidine, rilmenidine, verapamil),

·       other medicines used to treat high blood pressure, including angiotensin II receptor blocker (ARB), aliskiren (see also information under the headings “Do not take Cosyrel” and “Warnings and precautions”) or diuretics (medicines which increase the amount of urine produced by the kidneys),

·       potassium-sparing  drugs (e.g. triamterene, amiloride), potassium supplements or potassium-containing salt substitutes, other drugs which can increase potassium in your body (such as heparin, a medicine used to thin blood to prevent clots; trimethoprim and co-trimoxazole also known as trimethoprim/sulfamethoxazole for infections caused by bacteria),

·       potassium-sparing drugs used in the treatment of heart failure: eplerenone and spironolactone at doses between 12,5 mg to 50 mg by day,

·       sympathomimetics agents to treat clinical shock (adrenaline, noradrenaline, dobutamine, isoprenaline, ephedrine),

·       estramustine used in cancer therapy,

·       medicines, which are most often used to treat diarrhea (racecadotril) or avoid rejection of transplanted organs (sirolimus, everolimus, temsirolimus and other drugs belonging to the class of so-called mTor inhibitors). See section “Warnings and precautions”,

·       sacubitril/valsartan (used to treat long-term heart failure). See sections “Do not take Cosyrel” and “Warnings and precautions”.

·       lithium used to treat mania or depression,

·       certain medicines used to treat depression such as imipramine, amitriptylin, monoamine oxidase (MAO) inhibitors (except MAO-B inhibitors),

·       certain medicines used to treat schizophrenia (antipsychotics),

·       certain medicines used to treat epilepsy (phenytoin, barbiturates such as phenobarbital),

·       anaesthetic agents used for surgery,

·       vasodilators including nitrates (products that make the blood vessels become wider),

·       trimethoprim used to treat infections,

·       immunosuppressants (medicines which reduce the defence mechanism of the body) such as ciclosporin, tacrolimus, used for the treatment of auto-immune disorders or following transplant surgery,

·       allopurinol used to treat gout,

·       parasympathomimetics medicines used to treat conditions such as Alzheimer’s disease or glaucoma,

·       topical beta-blockers used to treat glaucoma (increase pressure in the eye),

·       mefloquine used to prevent or treat malaria,

·       baclofen used to treat muscle stiffness in diseases such as multiple sclerosis,

·       gold salts, especially with intravenous administration (used to treat symptoms of rheumatoid arthritis),

·       medicines to treat diabetes such as insulin, metformin, linagliptin, saxagliptin, sitagliptin, vildagliptin,

·       non-steroidal anti-inflammatory drugs (NSAIDs) such as ibuprofen or diclofenac or high dose of acetylsalicylic acidused to treat arthritis, headache, pain or inflammation.

 

Cosyrel with food, drink and alcohol

It is preferable to take Cosyrel before a meal.

 

Pregnancy and breast-feeding

If you are pregnant or breast-feeding, think you may be pregnant or are planning to have a baby, ask your doctor or pharmacist for advice before taking this medicine.

 

Pregnancy

You must tell your doctor if you think you are (or might become) pregnant. Your doctor will normally advise you to stop taking Cosyrel before you become pregnant or as soon as you know you are pregnant and will advise you to take another medicine instead of Cosyrel. Cosyrel is not recommended in early pregnancy, and must not be taken when more than 3 months pregnant, as it may cause serious harm to your baby if used after the third month of pregnancy.

Breast-feeding

Tell your doctor if you are breast-feeding or about to start breast-feeding. Cosyrel is not recommended for mothers who are breast-feeding, and your doctor may choose another treatment for you if you wish to breast-feed, especially if your baby is a newborn, or was born prematurely.

 

Driving and using machines

Cosyrel usually does not affect alertness but dizziness or weakness due to low blood pressure may occur in some patients, particularly at the start of treatment or upon change of medication as well as in conjunction with alcohol. If you are affected in this way, your ability to drive or to operate machinery may be impaired.

 

Cosyrel contains sodium

Cosyrel contains less than 1 mmol sodium (23 mg) per tablet, that is to say essentially ‘sodium-free’.

 


Always take this medicine exactly as your doctor or pharmacist has told you. Check with your doctor or pharmacist if you are not sure.

 

The recommended dose is one tablet once daily. Swallow your tablet with a glass of water in the morning before a meal.

 

In some cases, your doctor may prescribe one half tablet of Cosyrel once daily in the morning before a meal.

 

Patient with kidney disease

Cosyrel is not recommended if you suffer from moderate and severe kidney disease.

 

Use in children and adolescent

Use in children and adolescent is not recommended.

 

If you take more Cosyrel than you should

If you take more tablets than prescribed, contact you doctor or pharmacist immediately.

The most likely effect in case of overdose is low blood pressure which can make you feel dizzy or faint (if this happens, lying down with the legs raised can help), severe difficulty in breathing, tremors (due to decreaded blood sugar) and slow heart rate.

 

If you forget to take Cosyrel

It is important to take your medicine every day as regular treatment works better. However, if you forget to take a dose of Cosyrel, take the next dose at the usual time. Do not take a double dose to make up for a forgotten dose.

 

If you stop taking Cosyrel

Do not suddenly stop taking Cosyrel or change the dose without consulting your doctor since this can cause severe worsening of heart condition. Treatment should not be discontinued abruptly, especially in patients with coronary artery disease.

 

If you have any further questions on the use of this medicine, ask your doctor, pharmacist or nurse.

 


Like all medicines, this medicine can cause side effects, although not everybody gets them.

 

Stop taking the medicinal product and see a doctor immediately, if you experience any of the following side effects:

·       severe dizziness or fainting due to low blood pressure (common - may affect up to 1 in 10 people),

·       worsening of heart failure causing increased breathlessness and /or retention of fluid (common - may affect up to 1 in 10 people),

·       swelling of the face, lips, mouth, tongue or throat, difficulty in breathing (angioedema) (uncommon - may affect up to 1 in 100 people),

·       sudden wheeziness, chest pain, shortness of breath, or difficulty in breathing (bronchospasm) (uncommon - may affect up to 1 in 100 people),

·       unusual fast or irregular heart-beat, chest pain (angina) or heart attack (very rare - may affect up to 1 in 10,000 people),

·       weakness of arms or legs, or problems speaking which could be a sign of a possible stroke (very rare - may affect up to 1 in 10,000 people),

·       inflamed pancreas which may cause severe abdominal and back pain accompanied with feeling very unwell (very rare - may affect up to 1 in 10,000 people),

·       yellowing of the skin or eyes (jaundice) which could be a sign of hepatitis (very rare - may affect up to 1 in 10,000 people),

·       skin rash which often starts with red itchy patches on your face, arms or legs (erythema multiforme) (very rare - may affect up to 1 in 10,000 people).

 

Cosyrel is usually well tolerated but, as with any medicine, people can experience different side effects, particularly when treatment is first started.

 

If you notice any of the side effects listed below or any not listed, please tell your doctor or pharmacist immediately:

 

Very common (may affect more than 1 in 10 people):

·       slow heart beat.

 

Common (may affect up to 1 in 10 people):

·       headache,

·       dizziness,

·       vertigo,

·       taste disturbances,

·       pins and needles,

·       tingling or numbness of the hands of feet,

·       vision disturbances,

·       tinnitus (sensation of noises in the ears),

·       feeling of coldness in hands or feet,

·       cough,

·       shortness of breath,

·       gastro-intestinal disorders such as nausea, vomiting, abdominal pain, difficulty of digestion or dyspepsia, diarrhoea, constipation,

·       allergic reactions such as skin rashes, itching,

·       muscle cramps,

·       feeling of tiredness,

·       fatigue.

 

Uncommon (may affect up to 1 in 100 people):

·       mood swings,

·       sleep disturbances,

·       depression,

·       dry mouth,

·       intense itching or severe skin rashes,

·       formation of blister clusters over the skin,

·       increased sensitivity of the skin to sun (photosensitivity reaction),

·       sweating,

·       kidney problems,

·       impotence,

·       an excess of eosinophils (a type of white blood cells),

·       somnolence,

·       fainting,

·       palpitations,

·       tachycardia,

·       irregular heart rate (AV-conduction disturbances inflammation of blood vessels (vasculitis),

·       dizziness when standing up,

·       muscle weakness,

·       arthralgia (joint pain),

·       myalgia (muscle pain),

·       chest pain,

·       malaise,

·       localised swelling (oedema peripheral),

·       fever,

·       fall,

·       changes in laboratory parameters: high blood level of potassium reversible on discontinuation, low level of sodium, very low blood sugar level (hypoglycaemia) in case of diabetic patients, increased blood urea, increased blood creatinine.

 

Rare (may affect up to 1 in 1000 people):

·       acute renal failure,

·       dark urine, feeling sick (nausea) or being sick (vomiting), muscle cramps, confusion and seizures. These could be symptoms of a condition called SIADH (inappropriate antidiuretic hormone secretion),

·       decreased or absent urine output,

·       flushing,

·       nightmares, hallucinations,

·       reduced tear flow (dry eyes),

·       hearing problems,

·       inflammation of the liver which can cause yellowing of the skin or the whites of the eyes,

·       allergic runny nose, sneezing,

·       allergy-like reactions such as itching, flush, rash,

·       psoriasis worsening,

·       changes in laboratory parameters: increased level of liver enzymes, high level of serum bilirubin, fat levels differing from normal.

 

Very rare (may affect up to 1 in 10,000 people):

·       confusion,

·       irritation and redness of the eye (conjunctivitis),

·       eosinophilic pneumonia (a rare type of pneumonia),

·       inflammation of the pancreas (which causes severe pain in the abdomen and the back),

·       hair loss,

·       appearance or worsening of scaly skin rash (psoriasis), psoriasis-like rash,

·       changes in blood values such as a lower number of white and red blood cells, lower haemoglobin, lower number of blood platelets.

 

Frequency not known (frequency cannot be estimated from the available data):

·       discoloration, numbness and pain in fingers or toes (Raynaud’s phenomenon).

 

If you have these symptoms contact your doctor as soon as possible.

If you get any side effects, talk to your doctor or pharmacist or nurse. This includes any possible side effects not listed in this leaflet. You can also report side effects directly via the national reporting system. By reporting side effects you can help provide more information on the safety of this medicine.

 


Keep this medicine out of the sight and reach of children.

 

Do not use this medicine after the expiry date which is stated on the carton. The expiry date refers to the last day of that month.

 

This medicine does not require any special storage conditions.

 

Once opened, Cosyrel should be used within 20 days for tablet containers of 10 film-coated tablets, 60 days for tablet containers of 28 or 30 film-coated tablets and 100 days for tablet containers of 100 film-coated tablets.

 

Do not throw away any medicines via wastewater or household waste. Ask your pharmacist how to throw away medicines you no longer use. These measures will help protect the environment.


-      The active substances are bisoprolol fumarate and perindopril arginine. Each tablet of Cosyrel contains 5 mg bisoprolol fumarate equivalent to 4.24 mg bisoprolol and 10 mg perindopril arginine equivalent to 6.790  mg perindopril.

-      The other ingredients are cellulose microcrystalline PH 102 (E460), calcium carbonate (E170), pregelatinised maize starch, sodium starch glycolate type A (E468), silica colloidal anhydrous (E551), magnesium stearate (Liga E572), croscarmellose sodium (E468), glycerol (E422), Hypromellose (E464), Macrogol 6000, titanium dioxyde (E171), Iron oxide yellow (E172), Iron oxide red (E172) and water purified.


Cosyrel is a pink beige, oblong, bilayer scored film-coated tablet engraved with ‘ ’ on one face and ‘5/10’ on the other face. The tablet can be divided into equal halves. The tablets are available in box of 10, 28, 30, 84 (3 tablet containers of 28), 90 (3 tablet containers of 30), 100 and 120 (4 tablet containers of 30) film-coated tablets. Not all pack sizes may be marketed.

يحتوي كوسيريل على مادتين فعالتين، بيسوبرولول فومارات وبيريندوبريل أرجينين، في قرص واحد

•          ينتمي بيسوبرولول فومارات الى فئة من الأدوية تسمى حاصرات بيتا. يبطيء بيسوبرولول من سرعة القلب ويسمح للقلب بضخ الدم بكفاءة أكبر في جميع أنحاء الجسم.

•         بيريندوبريل أرجينين مثبط لإنزيم تحويل الأنجيوتنسين. وهو يعمل عن طريق توسيع الأوعية الدموية، مما يسهل عمل القلب في دفع الدم إلى الأوعية الدموية.

يستعمل كوسيريل لعلاج ضغط الدم المرتفع و / أو للحدّ من إمكانية إصابات القلب، كالذبحة الصدرية، لدى المرضى المصابين بمرض الشرايين التاجية المستقر (مع تناقص أو انقطاع تدفق الدم إلى عضلة القلب) والذين أصيبوا بذبحة صدرية و / أو  الذين خضعوا لإجراء توسيع أوعية القلب لتحسين تدفق الدم إلى القلب.


بدلا من تناول بيسوبرولول و بيريندوبريل كلا على حدة، ستتناول قرصا واحدا من كوسيريل يحتوي على كلا المادتين الفعالتين بنفس الجرعة.

 

لا تتناول كوسيريل ٥ ملغم / ١٠ ملغم ، أقراص ملبسة قابلة للكسر:

·       إذا كنت مصابا بالحساسية  تجاه بيسوبرولول أو أي من حاصرات بيتا الأخرى، أو تجاه بيريندوبريل أو أي من مثبطات الأنزيم المحوّل للأنجيوتنسين، أو أي من مكونات هذا الدواء الأخرى المذكورة في القسم ٦،

·       إذا كنت مصابا بقصور القلب الذي ساء فجأة و / أو يتطلب دخول المستشفى،

·       إذا سبق وأصبت بصدمة قلبية (مرض قلبي شديد بسبب شدة انخفاض ضغط الدم)،

·       إذا أصبت بحالة قلبية تتصف ببطء ضربات القلب أو عدم انتظامها (حصار أذيني بطيني من الدرجة الثانية أو الثالثة، حصار جيبي أذيني، مرض جيبي)

·       إذا كان نظم قلبك بطيئا،

·       إذا كان ضغط دمك منخفضا جدا،

·       إذا كنت مصابا بالربو الشديد أو بمشاكل تنفسية شديدة مزمنة،

·       إذا كنت مصابا باضطراب شديد في الدوران المحيطي (مثل متلازمة رينو)، التي قد تسبب نخزا في أصابع اليدين والقدمين أو تجعلها تبدو شاحبة أو زرقاء،

·       إذا كنت مصابا بالفيوكروموسيتوما غير المعالجة، وهي ورم نادر يصيب الغدة فوق الكلوية (اللب)،

·       إذا كنت مصابا بالحماض الاستقلابي، والذي يعني وجود كمية زائدة من الحمض في دمك،

·       إن كنت تعرضت أثناء علاج سابق بواسطة مثبطات الإنزيم المحول  للأنجيوتنسين لأعراض مثل الصفير أثناء التنفس، أو تورم الوجه أو اللسان أو الحنجرة، أو إن أصبت أنت أو أحد أفراد أسرتك بهذه الأعراض في أي ظرف أخر (الوذمة الوعائيّة)،

·       إذا كنتِ حاملا منذ أكثر من ٣ شهور (كما يفضل تجنب تناول كوسيريل في مرحل الحمل المبكرة،  راجع قسم "الحمل")،

·       إذا كنت مصابا بالسكري أو القصور الكلوي وتتلقى علاجا يحتوي على أليسكيرين لخفض ضغط الدم،

·       إذا كنت تخضع للديلزة أو أي نوع آخر من عمليات تنقية الدم. قد يكون كوسيمبريل غير مناسب لك اعتمادا على الألة المستخدمة،

·       إذا كنت تعاني من مشاكل كلوية حيث يتناقص ورود الدم إلى الكليتين (تضيّق الشريان الكلوي)،

·       إذا كنت قد تلقيت أو تتلقى علاجا بـ ساكوبتريل/فالسارتان، وهو دواء لعلاج قصور القلب نظرا لازدياد إمكانية حدوث الوذمة الوعائية (تورم سريع تحت الجلد في منطقة كالبلعوم) (راجع قسمي "تحذيرات واحتياطات" و "الأدوية الأخرى وكوسيمبريل").

 

تحذيرات واحتياطات

تحدث إلى  طبيبك، الصيدلاني أو الممرضة قبل تناول كوسيريل ٥ ملغم / ١٠ ملغم أقراص ملبسة قابلة للكسر.

 

·       إذا كنت مصابا بالسكري.

·       إذا كنت تعاني من مرض كلوي (بما في ذلك نقل الكلية) أو إذا كنت تخضع للديلزة.

·       إذا كنت مصابا بمرض كبدي.

·       إذا كنت مصابا بتضيق الأورطي أو التاجي (تضيق الشريان الرئيسي المغذي لعضلة القلب) أو باعتلال القلب الضخامي (مرض يصيب عضلة القلب) أو بتضيق الشريان الكلوي (تضيق الشريان الذي يزوّد الكليتين بالدم).

·       إذا كانت لديك معدلات مرتفعة بشكل غير طبيعي من هرمون الألدوستيرون في دمك (الأدوستيرونيّة الأوليّة).

·       إذا كنت مصابا بقصور القلب أو أي مشكلة أخرى في القلب مثل اضطرابات نظم القلب أو بألم شديد في الصدر أثناء الراحة (خناق برنزميتال).

·       إذا كنت تعاني من مرض كولاجيني (مرض النسيج الضام) مثل الذئبة الحمامية الجهازية أو تصلب الجلد.

·       إذا كنت تتبع حمية غذائية منخفضة الملح أو تستعمل بدائل الملح الحاوية على البوتاسيوم (الكميات المرتفعة من البوتاسيوم في الدم قد تسبب تغيرات في سرعة القلب).

·       إذا أصبت مؤخرا بالإسهال والتقيؤ، أو كنت مصابا بالجفاف (قد يسبب كوسيريل انخفاضا في ضغط الدم).

·       إذا كنت ستخضع لفصادة البروتين الشحمي المنخفض الكثافة (LDL) (أي عمليّة آليّة لسحب الكوليسترول من الدم).

 

 

·       إذا كنت تخضع أو ستخضع لعلاج إزالة التحسّس لتخفيف تأثير لدغات النحل والدبابير.

·       إن كنت تخضع لنظام أو حمية غذائية صارمة.

·       إذا كنت ستخضع لتخدير طبي و / أو عملية جراحية.

·       إذا كنت تعاني من مشاكل في الدورة الدموية في الأطراف.

·       إذا كنت مصابا بالربو أو بمرض رئوي مزمن.

·       إذا كنت مصابا (أو أصبت) بالصدفية.

·       إذا كنت مصابا بورم في الغدد فوق الكلوية (فيوكروموسيتوما).

·       إذا كنت مصابا بمشاكل في الغدة الدرقية (يستطيع كوسيريل أن يقنّع أعراض فرط نشاط الدرق).

·       إذا كنت مصابا بوذمة وعائية (تفاعلات تحسسية خطيرة تترافق بتورّم الوجه، الشفتين، اللسان أو الحلق قد تؤدي إلى صعوبة في البلع أو التنفس). وهذا قد يحدث في أي وقت أثناء العلاج. إذا كنت تعاني من أعراض كهذه، توقف عن تناول كوسيريل واتصل بطبيبك على الفور.

·       إذا كنت من العرق الأسود: قد يسبب هذا الدواء ارتفاعا في إمكانية الإصابة بالوذمة الوعائية وقد يكون أقل فعالية في خفض ضغط دمك مقارنة بالمرضى من غير العرق الأسود،

·       إذا كنت تتناول ايا من الأدوية التالية لعلاج ارتفاع ضغط الدم:

o      أحد حاصرات مستقبلات الأنجيوتنسين ΙΙ (التي تعرف أيضا باسم مجموعة السارتان – مثل فالسارتان، تلميسارتان، إربيسارتان)، وخاصة إذا كنت تشكو من اضطرابات كلوية ذات صلة بالسكري،   

o      أليسكيرين.

 

 قد يقوم طبيبك بمراقبة وظيفتك الكلوية، ضغط الدم، وكمية الشوارد الكهربائية (مثل البوتاسيوم) في دمك. 

 

·       إذا كنت تتناول ايا من الأدوية التالية، فإن أمكانية الإصابة بالوذمة الوعائية ترتفع:

o       راسيكادوتريل (الذي يستعمل لعلاج الإسهال)،

o       سيروليموس، إيفيروليموس، تمسيروليموس والأدوية الأخري التي تنتمي إلى مجموعة الأدوية التي تدعى مثبطات الهدف الميكانيكي لرابامايسين m-TOR inhibitors (التي تستعمل لتفادي رفض الأعضاء المزروعة ولعلاج السرطان) ،

o       ساكوبتريل  (الذي يتوفر بتركيبة ثابتة الجرعة مع فالسارتان)، الذي يستعمل لعلاج قصور القلب المزمن.

o      ليناغلبتين، ساكساغلبتين، سيتاغلبتين، فيلداغلبتين، وكافة الأدوية الأخرى التي تنتمي إلى مجموعة غلبتين (التي تستعمل لعلاج السكري).

 

 

لا تتوقف عن تناول كوسيريل بشكل مفاجيء، لأن هذا قد يؤدي إلى تدهور وظيفة القلب. يجب عدم إيقاف العلاج بواسطة كوسيريل بشكل مفاجيء، وخاصة لدى المرضى الذين يعانون من مرض الشريان التاجي.
 

يجب أن تعلمي طبيبك إذا كنت تعتقدين (أو إذا كنت تتوقعين) أنك حامل. لا يُنصح بتناول كوسيريل في مراحل الحمل المبكرة، ولا يجوز تناوله على الإطلاق بعد الشهر الثالث للحمل، لأن تناوله في هذه المرحلة قد يسبب إصابات خطيرة على طفلك (راجعي قسم "الحمل").

 

الأطفال والمراهقون

لا ينصح باستعمال كوسيريل للأطفال والمراهقين الذين لم يبلغوا عمر ١٨ سنة.


الأدوية الأخرى وكوسيريل ٥ ملغم / ١٠ ملغم ، أقراص ملبسة قابلة للكسر


أعلم طبيبك إذا كنت تتناول حالياً أو تناولت مؤخّراً أو قد تتناول أي دواء آخر. 


قد تؤثر بعض الأدوية على فعالية كوسيريل أو قد تتغير فعاليتها بسبب كوسيريل.


هذا النوع من التداخل قد يجعل الدواء (الأدوية) أقل فاعلية، أو قد يؤدي أحيانا إلى زيادة مخاطر أو شدة التأثيرات الجانبية.

تحدث إلى طبيبك إذا كنت تتناول أيا من الادوية التالية: 

  • الأدوية المستعملة لخفض ضغط الدم أو لعلاج مشاكل القلب (مثل أميودارون، أملوديبين، كلونيدين، ديجيتال، ديلتيازيم، ديزوبيراميد، فيلوديبين، فليكاينيد، ليدوكايين، مثيل دوبا، موكسونيدين، بروكايين أميد، بروبافينون، كينيدين، ريلمينيدين، فيراباميل).
  • الأدوية الأخرى الخافضة لضغط الدم، بما فيها حاصرات مستقبلات الأنجيوتنسين ΙΙ أو أليسكيرين (راجع أيضا المعلومات الواردة تحت عنوان "لا تتناول كوسيريل" و"تحذيرات واحتياطات")، أو مدرّات البول (أي الأدوية التي ترفع مقدار البول الناتج عن الكليتين).

·       المدرّات الموفّرة للبوتاسيوم ( تريامتيرين، أميلوريد)، مكملات البوتاسيوم أو بدائل الملح المحتوية على البوتاسيوم، الأدوية الأخرى التي يمكنها زيادة معدّل البوتاسيوم في جسمك (مثل الهيبارين الذي يستعمل لتمييع الدم ومنع التخثر؛ وترايميثوبريم وكوتريموكسازول المعروف أيضا باسم ترايميثوبريم/سلفاميثوكسازول لعلاج حالات العدوى البكترية)،

  • المدرّات الموفّرة للبوتاسيوم والمستعملة في علاج قصور القلب: إيبليرينون وسبيرونولاكتون بجرعات تتراوح بين ١٢٫٥٠ ملغم و٥٠ ملغم يوميا.
  • العوامل المحاكية للودّي المستعملة في علاج الصدمة (أدرنالين، نورأدرنالين، دوبيوتامين، إيزوبرنالين، إفدرين).
  • إسترامُستين (الذي يستعمل لعلاج السرطان).
  • الأدوية التي كثيرا ما تستعمل لعلاج الإسهال (راسيكادوتريل) أو تفادي رفض الأعضاء المزروعة (سيروليموس، إيفيروليموس، تيمسيروليموس وأدوية أخرى من الأدوية التي تدعى مثبطات m-Tor). راجع قسم "تحذيرات واحتياطات"،
  • ساكوبتريل / فالسارتان (الذي يستعمل لعلاج قصور القلب المزمن). راجع قسمي "لا تتناول كوسيريل" و "تحذيرات واحتياطات".
  • الليثيوم المُستعمل في علاج الذهان الهَوَسي الاكتئابي.
  • الأدوية المستعملة في علاج الاكتئاب مثل أيميبرامين، أميتريبتيلين، مثبطات  أكسيداز أحادي الأمين (MAOIs) باستثناء مثبط أكسيداز أحادي الأمين- ب. 
  • بعض الأدوية المستعملة في علاج الفصام (مضادات الذهان).
  • بعض الأدوية المستعملة في علاج الصرع (فنيتوين، باربيتورات مثل فينوباربيتال).
  • ادوية التخدير المستعملة في الإجراءات الجراحية.
  • موسّعات الأوعية الدموية بما فيها النترات (المستحضرات التي من شأنها توسيع الأوعية الدمويّة).
  • ترايميثوبريم (لعلاج حالات العدوى).  
  • كابتات المناعة (الأدوية المخفّضة لآليّات المناعة في الجسم) المستعملة في علاج أمراض المناعة الذاتيّة أو بعد عمليّة زرع الأعضاء (سيكلوسبورين، تاكروليموس). 
  • آلوبورينول (لعلاج النقرس).
  • الأدوية المحاكية للّاودّي، مثل تلك المستعملة لعلاج داء ألزهايمر أو الزَرَق (الغلوكوما).
  • حاصرات بيتا الموضعية لعلاج الزَرَق (ارتفاع الضغط داخل العين).
  • ميفلوكين المستعمل في الوقاية من الملاريا وعلاجها.
  • باكلوفين المستعمل في علاج التصلب العضلي في أمراض مثل التصلب اللويحي.
  • أملاح الذهب، وخاصة التي تعطى داخل الوريد (والتي تستعمل لعلاج أعراض التهاب المفاصل الروماتويدي). 
  • الأدوية المستعملة في علاج السكري مثل الانسولين، متفورمين، ليناغلبتين، ساكساغلبتين، سيتاغلبتين، فيلداغلبتين.
  • مضادات الالتهاب غير الستيرويديّة (مثل الإيبوبروفين أو ديكلوفيناك أو الجرعات المرتفعة من الأسبيرين  حمض الأستيل ساليسيليك) التي تستعمل لعلاج التهاب المفاصل، الصداع، الألم أو الالتهاب.

 

كوسيريل ٥ ملغم / ١٠ ملغم ، أقراص ملبسة قابلة للكسر مع الطعام والشراب

ينصح بتناول كوسيريل قبل وجبة الطعام.

الحمل والإرضاع

إذا كنت حاملا أو مرضعا، أو تعتقدين بأنك حامل أو تخططين للإنجاب، عليكِ باستشارة طبيبك أو الصيدلاني قبل المباشرة بتناول هذا الدواء.

الحمل 
يجب أن تبلغي طبيبك إذا كنت تعتقدين بأنك حامل (أو أنك قد تصبحين حاملًا). سينصحك طبيبك في الأحوال الطبيعية بالتوقف عن تناول كوسيريل قبل حدوث الحمل أو بمجرد أن تعلمي بحدوثه. وسينصحك بتناول دواء آخر بدلا عن كوسيريل. لا يُنصح بتناول كوسيريل في مراحل الحمل المبكرة، ولا يجوز تناوله على الإطلاق بعد الشهر الثالث للحمل، لأن تناوله في هذه المرحلة قد يسبب إصابات خطيرة على طفلك. 

الإرضاع
أخبري طبيبك إن كنت مرضعا أو إن كنت سوف تباشرين بالإرضاع.

لا تُنصح المرضعات بتناول كوسيريل، وقد يختار طبيبك علاجا آخر إذا كنت تريدين الإرضاع، وخاصة إن كان الطفل حديث الولادة أو خديجا.

 

قيادة السيارات وتشغيل الآليات

لا يؤثر كوسيريل على اليقظة، ولكن قد يظهر شعور بالدوار أو بالتعب لدى بعض المرضى نتيجة هبوط ضغط الدم، وخاصة عند بدء العلاج أو في حال تغيير العلاج، بالإضافة إلى حالات تناول الكحول. لذا، فقد تتأثر قدرتك على قيادة السيارات أو تشغيل الآلات.

 

يحتوي كوسيريل ٥ ملغم / ١٠ملغم، أقراص ملبسة قابلة للكسر  على الصوديوم 

يحتوي كوسيريل على أقل من ١ ميليمول (٢٣ ملغ) من الصوديوم في كل حبّة، مما يعني أنه بالأساس "خال من الصوديوم".

تناول هذا الدواء ملتزما بالطريقة التي وصفها لك طبيبك أو الصيدلاني. ان كان لديك شك، فاسأل طبيبك أو الصيدلاني.


الجرعة
يبلغ مقدار الجرعة الموصى بها  قرص واحد يوميا. يجب ابتلاع القرص مع كأس من الماء صباحا قبل وجبة الإفطار.

في بعض الحالات، قد يصف لك طبيبك نصف قرص من كوسيريل مرة واحدة يوميا يؤخذ صباحا قبل الإفطار.

القصور الكلوي

لا يوصى يبتناول كوسيريل إذا كنت مصابا بقصور كلوي شديد.


الاستعمال لدى الأطفال والمراهقين

لا يوصى باستعماله لدى الأطفال والمراهقين.

إذا تناولت أكثر مما ينبغي من كوسيريل ٥ ملغم / ١٠ ملغم أقراص ملبسة قابلة للكسر:

استشر طبيبك أو الصيدلاني على الفور.

فأكثر ما يُحتمل وقوعه في حال الإفراط بالجرعة، هو هبوط الضغط الشرياني، الذي قد يسبب الدوار أو الإغماء (إن حدث هذا عليك بالاستلقاء مع رفع ساقيك للأعلى)، صعوبة التنفس، رجفان (بسبب هبوط سكر الدم) وبطء سرعة القلب.

إذا نسيت أن تتناول كوسيريل ٥ ملغم / ١٠ ملغم أقراص ملبسة قابلة للكسر:

من المهم أن تتناول دواءك كل يوم لأن العلاج المنتظم أكثر فعالية. ولكن إذا نسيت تناول جرعة من كوسيريل، عليك ببساطة متابعة علاجك في اليوم التالي كالمعتاد. لا تتناول جرعة مضاعفة للتعويض عن الجرعة التي نسيت تناولها.

 

إذا توقفت عن تناول كوسيريل ٥ ملغم / ١٠ ملغم أقراص ملبسة قابلة للكسر:

لا تتوقف فجأة عن تناول كوسيريل أو تغير الجرعة دون اسستشارة طبيبك لأن مشاكل قلبك قد تسوء.  يجب عدم إيقاف العلاج بشكل مفاجئ، خاصة

لدى المرضى المصابين بمرض الشريان التاجي.

إذا كانت لديك أي أسئلة إضافية بخصوص استعمال هذا الدواء، فاستشر طبيبك، الصيدلاني أو الممرض/ـة للحصول على المزيد من المعلومات.

كما هي الحال مع كافة الأدوية، من الممكن أن يسبب كوسيريل تأثيرات جانبية، رغم أنها لا تصيب كافة الأشخاص.

 

 

إن لاحظت حدوث أيّ من التأثيرات الجانبيّة التالية، توقف فورا عن تناول هذا الدواء واتصل بطبيبك مباشرة:

·       دوار شديد أو إغماء بسبب هبوط ضغط الدم (شائع - قد يصيب حتى ١ من ١٠ مرضى)،

·       تدهور قصور القلب، مما يسبب ازدياد قصر النفس و / أو احتباس الماء، (شائع - قد يصيب حتى ١ من ١٠ مرضى)،

·       تورم الوجه، الشفتين، الفم، اللسان أو الحلق، صعوبة التنفس (وذمة وعائية) (غير شائعة - قد تصيب حتى ١ من كل ١٠٠ مريض)،

·       بدء مفاجئ لصفير الصدر، ألم الصدر، ضيق النفس أو صعوبات تنفسية (تشنج قصبي) (غير شائعة - قد تصيب حتى ١ من كل ١٠٠ مريض)،

·       ضربات قلب سريعة أو غي منتظمة بشكل غير اعتيادي، ألم الصدر (ذبحة صدرية) أو احتشاء عضلة القلب (نادرة جدا - قد تصيب حتى ١ من كل ١٠٠٠٠ مريض)،

·       ضعف في الذراعين أو الساقين، أو مشاكل عند التكلم قد تكون علامة لسكتة دماغية ممكنة الحدوث (نادر جدا - قد تصيب حتى ١ من كل ١٠٠٠٠ مريض)،

·       التهاب في البنكرياس، يمكن أن يؤدي إلى ألم شديد في البطن والظهر مترافق بتوعك شديد (نادر جدا - قد تصيب حتى ١ من كل ١٠٠٠٠ مريض)،

·       اصفرار الجلد أو العينين (يرقان) الذي قد يكون علامة لالتهاب الكبد (نادر جدا - قد تصيب حتى ١ من كل ١٠٠٠٠ مريض)،

·       طفح كثيرا ما يبدأ ببقع حمراء حاكة على الوجه، الذراعين أو الساقين (حمامى متعددة الأشكال) (نادرة جدا - قد تصيب حتى ١ من كل ١٠٠٠٠ مريض)،

يعتبر كوسيريل جيد التحمل عادة، ولكنه مثل أي دواء آخر، قد يسبب تأثيرات جانبية مختلفة، خاصة عند بدء العلاج.

 

إذا لاحظت حدوث أي من التأثيرات الجانبية التالية، فاتصل بطبيبك أو الصيدلاني على الفور:

شائعة جدا (قد تصيب أكثر من ١ من ١٠ مرضى):

·       بطء ضربات القلب


شائعة (قد يصيب حتى ١ من ١٠ مرضى):

·       صداع،

·       دوار،

·       دوخة،

·       اضطراب حسّ التذوّق،

·       شعور بالنخز،

·       شعور بالنخز أو الاخدرار في أخمص القدمين،

·       اضطرابات بصرية،

·       طنين (شعور بالضجيج في الأذنين)،

·       شعور ببرودة اليدين والقدمين،

·       سعال،

·       صعوبة التنفس،

·       اضطرابات معدية معوية (غثيان، تقيؤ، ألم في البطن، صعوبة الهضم أو سوء الهضم، إسهال، إمساك)،

·       تفاعلات تحسسية (كالطفح، الحكة)،

·       تشنجات عضلية،

·       شعور بالتعب،

·       إعياء.

 

غير شائعة (قد تصيب حتى ١ من ١٠٠ مريض):

·       اضطرابات مزاجية،

·       صعوبات في النوم،

·       اكتئاب،

·       جفاف الفم،

·       حكة شديدة أو طفح جلدي شديد،

·       تشكل فقاعات جلدية،

·       ازدياد حساسية الجلد للشمس (تفاعل تحسسي ضوئي)،

·       تعرّق،

·       مشاكل في الكلى،

·       عنانة،

·       ازدياد معدل اليوزينيات في الدم (نوع من خلايا الدم البيضاء)،

·       نعاس،

·       إغماء،

·       خفقان،

·       تسرع ضربات القلب،

·       عدم انتظام ضربات القلب (اضطراب في النقل الأذيني البطيني، التهاب الأوعية الدموية)،

·       دوخة عند التبول بوضعية الوقوف

·       ضعف عضلي،

·       ألم المفاصل،

·       ألم عضلي،

·       ألم الصدر،

·       انزعاج

·       تورم موضعي (وذمة محيطية)،

·       حمى،

·       سقوط،

·       تغيّر في المعايير المخبرية: ارتفاع معدّل البوتاسيوم في الدم القابل للتراجع بعد إيقاف الدواء، انخفاض معدّل الصوديوم، انخفاض شديد في معدّل سكر الدم لدى المرضى المصابين بالسكري، ارتفاع معدّل الكرياتينين في الدم، ارتفاع معدّل اليوريا في الدم.

 

نادرة (قد تصيب حتى ١ من ١٠٠٠ مريض):

·       قصور كلوي حادّ،

·       بول مركّز (داكن اللون)، غثيان أو تقيؤ، تشنج عضلي، ارتباك ونوبات. هذه الأعراض قد تكون ناجمة عن متلازمة إفراز غير مناسب من الهرمون المضاد للإدرار.

·       تناقص أو انقطاع إنتاج البول،

·       احمرار الوجه الاحتقاني،

·       كوابيس، هلاوس،

·       تناقص إفراز الدمع (جفاف العينين)،

·       اضطراب السمع،

·       التهاب الكبد الذي قد يسبب اصفرار الجلد أو القسم الأبيض من العيينين،

·       التهاب الأنف التحسسي، عطاس،

·       تفاعلات تحسسية مثل الحكة، الهبات  الساخنة، الطفح،

·       تفاقم الصدفية

·       تغيّر في المعايير المخبرية: ارتفاع معدّل الخمائر الكبدية، ارتفاع معدّل بيليروبين المصل، ارتفاع معدلات الكوليسترول عن الحدّ الطبيعي.

 

نادرة جدا (قد تصيب حتى ١ من ١٠٠٠٠ مريض):

  • ارتباك،
  • تخرش واحمرار العينين (التهاب الملتحمة)،
  • التهاب الرئة اليوزينيي (نوع نادر من التهاب الرئة)،
  • التهاب البنكرياس (الذي قد يسبب ألما في البطن والظهر)،
  • سقوط الشعر،
  • ظهور أو تدهور التهاب الجلد التوسفي (الصدفية)، اندفاعات صدفية الشكل
  • تبدّلات في مقاييس الدم مثل تناقص أعداد الكريات البيضاء والحمراء، انخفاض الهيموغلوبين، تناقص عدد الصفيحات الدموية.

 

معدّل حدوث غير محدد (لا يمكن تقديره استنادا إلى البيانات المتاحة)

·     تبقع، خدر وألم على مستوى اصابع اليدين والقدمين (متلازمة رينو).

 

إذا أصبت بهذه الأعراض فاتصل بطبيبك على الفور.

 

 

الإبلاغ عن التأثيرات الجانبية

إذا أصبت بأي تأثيرات جانبية، تحدث إلى <طبيبك> <أو>  <، >  <الصيدلاني> <أو الممرض / ـة>. وهذا يشمل أي تأثيرات جانبية ممكنة الحدوث وإن لم يرد ذكرها في هذه النشرة. كما يمكنك أيضا الإبلاغ عن التأثيرات الجانبية مباشرة عن طريق نظام الإبلاغ الوطني: للوكالة الوطنية لسلامة الأدوية والمنتجات الصحية (ANSM) والمراكز الإقليمية للتيقظ الدوائي – الموقع الالكتروني: www.ansm.sante.fr.

لا يتطلب هذا الدواء شروطا خاصة لحفظه.

لا تتخلص من أي مواد دوائية في مياه المجاري العامة أو مع قمامة المنزل. اسأل الصيدلاني عن طريقة التخلص من الأدوية التي لم تعد بحاجة إليها.  هذه  الإجراءات تساعد على حماية البيئة.

يتوفر هذا الدواء بشكل أقراص ملبسة بيج زهرية اللون، بيضاوية، ثنائية الطبقة، قابلة للكسر، نقش على أحد وجهيها و 10/5 على الوجه الآخر.  يمكن كسر هذه الأقراص إلى جرعتين متساويتين.

تتوفر هذه الأقراص في علب تحتوي على ١٠،٢٨، ٣٠، ٨٤ (٣ حاويات أقراص يحتوي كل منها على ٢٨ قرصا)، ٩٠قرصا (٣ حاويات أقراص يحتوي كل منها على ٣٠ قرصا) و١٠٠، ١٢٠ قرصا (٤ حاويات أقراص يحتوي كل منها على ٣٠ قرصا).

 Read this leaflet carefully before you start using this product as it contains important information for you

Cosyrel 5 mg/10 mg film-coated tablets

One film-coated tablet contains 5 mg of bisoprolol fumarate (equivalent to 4.24 mg bisoprolol) and 10 mg of perindopril arginine (equivalent to 6.790 mg perindopril). For the full list of excipients, see section 6.1.

Film-coated tablet Pink beige, oblong, bilayer scored film-coated tablet of 9.8 mm length and 5.4 mm width, engraved with ‘ ’ on one face and ‘5/10’ on the other face. Cosyrel 5 mg/10 mg scored tablet can be divided into equal doses.

Cosyrel is indicated as substitution therapy for treatment of hypertension and/or stable coronary artery disease (in patients with a history of myocardial infarction and/or revascularisation) in adult patients adequately controlled with bisoprolol and perindopril given concurrently at the same dose level.


Posology

 

The usual posology is one tablet once daily.

Patients should be stabilized with bisoprolol and perindopril at the same dose level for at least 4 weeks. The fixed dose combination is not suitable for initial therapy.

 

For patients stabilized with bisoprolol 2.5 mg and perindopril 5 mg: one half 5 mg/10 mg tablet once daily.

 

If a change of posology is required, titration should be done with the individual components.

 

Special population

 

Renal impairment (see section 4.4 and 5.2)

In patients with renal impairment, the recommended dose of Cosyrel 5 mg/10 mg should be based on creatinine clearance as outlined in table 1 below:

Cosyrel should be administered according to the renal function.

 

Paediatric population

The safety and efficacy of Cosyrel in children and adolescents have not been established. No data are available. Therefore, use in children and adolescents is not recommended.

 

Method of administration

 

Cosyrel tablet should be taken as a single dose once daily in the morning before a meal.


• Hypersensitivity to the active substances, or to any of the excipients listed in section 6.1, or to any other angiotensin converting enzyme (ACE) inhibitor • Acute heart failure or during episodes of heart failure decompensation requiring i.v. inotropic therapy • Cardiogenic shock • Second or third degree AV block (without pacemaker) • Sick sinus syndrome • Sinoatrial block • Symptomatic bradycardia • Symptomatic hypotension • Severe bronchial asthma or severe chronic obstructive pulmonary disease • Severe forms of peripheral arterial occlusive disease or severe forms of Raynaud's syndrome • Untreated phaeochromocytoma (see section 4.4) • Metabolic acidosis • History of angioedema associated with previous ACE inhibitor therapy (see section 4.4) • Hereditary or idiopathic angioedema • Second and third trimesters of pregnancy (see sections 4.4 and 4.6) • Concomitant use of Cosyrel with aliskiren-containing products in patients with diabetes mellitus or renal impairment (GFR < 60 ml/min/1.73m²) (see sections 4.4, 4.5 and 5.1), • Concomitant use with sacubitril/valsartan therapy. Cosyrel must not be initiated earlier than 36 hours after the last dose of sacubitril/valsartan (see sections 4.4 and 4.5), • Extracorporeal treatments leading to contact of blood with negatively charged surfaces (see section 4.5), • Significant bilateral renal artery stenosis or stenosis of the artery to a single functioning kidney (see section 4.4).

All warnings and precautions for use related to each component are applicable to Cosyrel.

 

Hypotension:

ACE inhibitors may cause a fall in blood pressure. Symptomatic hypotension is seen rarely in uncomplicated hypertensive patients and is more likely to occur in patients who have been volume-depleted e.g. by diuretic therapy, dietary salt restriction, dialysis, diarrhoea or vomiting, or who have severe renin-dependent hypertension (see sections 4.5 and 4.8). In patients with symptomatic heart failure, with or without associated renal insufficiency, symptomatic hypotension has been observed. This is most likely to occur in those patients with more severe degrees of heart failure, as reflected by the use of high doses of loop diuretics, hyponatraemia or functional renal impairment. In patients at increased risk of symptomatic hypotension, initiation of therapy and dose adjustment should be closely monitored. Similar considerations apply to patients with ischaemic heart or cerebrovascular disease in whom an excessive fall in blood pressure could result in a myocardial infarction or cerebrovascular accident.

If hypotension occurs, the patient should be placed in the supine position and, if necessary, should receive an intravenous infusion of sodium chloride 9 mg/ml (0.9%) solution. A transient hypotensive response is not a contraindication to further doses, which can be given usually without difficulty once the blood pressure has increased after volume expansion.

In some patients with congestive heart failure who have normal or low blood pressure, additional lowering of systemic blood pressure may occur with perindopril. This effect is anticipated and is usually not a reason to discontinue treatment. If hypotension becomes symptomatic, a reduction of dose or gradual discontinuation of treatment, using the individual components, may be necessary.

 

Hypersensitivity/Angioedema:

Angioedema of the face, extremities, lips, mucous membranes, tongue, glottis and/or larynx has been reported rarely in patients treated with ACE inhibitors, including perindopril (see section 4.8). This may occur at any time during therapy. In such cases, Cosyrel should promptly be discontinued. Therapy with beta-blocker must be continued. Appropriate monitoring should be initiated and continued until complete resolution of symptoms has occurred. In those instances where swelling was confined to the face and lips the condition generally resolved without treatment, although antihistamines have been useful in relieving symptoms.

Angioedema associated with laryngeal oedema may be fatal. Where there is involvement of the tongue, glottis or larynx, likely to cause airway obstruction, emergency therapy should be administered promptly. This may include the administration of adrenaline and/or the maintenance of a patent airway. The patient should be under close medical supervision until complete and sustained resolution of symptoms has occurred.

Patients with a history of angioedema unrelated to ACE inhibitor therapy may be at increased risk of angioedema while receiving an ACE inhibitor (see section 4.3).

Intestinal angioedema has been reported rarely in patients treated with ACE inhibitors. These patients presented with abdominal pain (with or without nausea or vomiting); in some cases there was no prior facial angioedema and C-1 esterase levels were normal. The angioedema was diagnosed by procedures including abdominal CT scan, or ultrasound or at surgery and symptoms resolved after stopping the ACE inhibitor. Intestinal angioedema should be included in the differential diagnosis of patients on ACE inhibitors presenting with abdominal pain.

 

The combination of perindopril with sacubitril/valsartan is contraindicated due to the increased risk of angioedema (see section 4.3). Sacubitril/valsartan must not be initiated until 36 hours after taking the last dose of perindopril therapy. If treatment with sacubitril/valsartan is stopped, perindopril therapy must not be initiated until 36 hours after the last dose of sacubitril/valsartan (see sections 4.3 and 4.5).

Concomitant use of ACE inhibitors with NEP inhibitors (e.g. racecadotril), mTOR inhibitors (e.g. sirolimus, everolimus, temsirolimus) and gliptins (e.g. linagliptin, saxagliptin, sitagliptin, vildagliptin) may lead to an increased risk of angioedema (e.g. swelling of the airways or tongue, with or without respiratory impairment) (see section 4.5). Caution should be used when starting racecadotril, mTOR inhibitors (e.g. sirolimus, everolimus, temsirolimus) and gliptins (e.g. linagliptin, saxagliptin, sitagliptin, vildagliptin) in a patient already taking an ACE inhibitor.

 

Hepatic failure:

Rarely, ACE inhibitors have been associated with a syndrome that starts with cholestatic jaundice and progresses to fulminant hepatic necrosis and (sometimes) death. The mechanism of this syndrome is not understood. Patients receiving ACE inhibitors who develop jaundice or marked elevations of hepatic enzymes should discontinue the ACE inhibitor and receive appropriate medical follow-up (see section 4.8).

 

Race:

ACE inhibitors cause a higher rate of angioedema in black patients than in non-black patients.

As with other ACE inhibitors, perindopril may be less effective in lowering blood pressure in black people than in non-blacks, possibly because of a higher prevalence of low-renin states in the black hypertensive population.

 

Cough:

Cough has been reported with the use of ACE inhibitors. Characteristically, the cough is non-productive, persistent and resolves after discontinuation of therapy. ACE inhibitor induced cough should be considered as part of the differential diagnosis of cough.

 

Hyperkalaemia:

Elevations in serum potassium have been observed in some patients treated with ACE inhibitors, including perindopril, ACE inhibitors can cause hyperkalaemia because they inhibit the release of aldosterone. The effect is usually not significant in patients with normal renal function. Risk factors for the development of hyperkalemia include those with renal insufficiency, worsening of renal function, age (> 70 years), diabetes mellitus, intercurrent events, in particular dehydration, acute cardiac decompensation, metabolic acidosis and concomitant use of potassium-sparing diuretics (e.g. spironolactone, eplerenone, triamterene, or amiloride), potassium supplements or potassium-containing salt substitutes; or those patients taking other drugs associated with increases in serum potassium (e.g. heparin, co-trimoxazole also known as trimethoprim/sulfamethoxazole) and especially aldosterone antagonists or angiotensin-receptor blockers. The use of potassium supplements, potassium-sparing diuretics, or potassium-containing salt substitutes particularly in patients with impaired renal function may lead to a significant increase in serum potassium. Hyperkalemia can cause serious, sometimes fatal arrhythmias. Potassium-sparing diuretics and angiotensin-receptor blockers should be used with caution in patients receiving ACE inhibitors, and serum potassium and renal function should be monitored. If concomitant use of the above-mentioned agents is deemed appropriate, they should be used with caution and with frequent monitoring of serum potassium (see section 4.5).

 

Combination with lithium:

The combination of lithium and perindopril is generally not recommended (see section 4.5).

 

Combination with potassium sparingdrugs, potassium supplements or potassium-containing salt substitutes:

The combination of perindopril and potassium sparing drugs, potassium supplements or potassium-containing salt substitutes is generally not recommended (see section 4.5).

 

Dual blockade of the renin-angiotensin-aldosterone system (RAAS):

There is evidence that the concomitant use of ACE-inhibitors, angiotensin II receptor blockers or aliskiren increases the risk of hypotension, hyperkalaemia and decreased renal function (including acute renal failure). Dual blockade of RAAS through the combined use of ACE-inhibitors, angiotensin II receptor blockers or aliskiren is therefore not recommended (see sections 4.5 and 5.1).

If dual blockade therapy is considered absolutely necessary, this should only occur under specialist supervision and subject to frequent close monitoring of renal function, electrolytes and blood pressure.

ACE-inhibitors and angiotensin II receptor blockers should not be used concomitantly in patients with diabetic nephropathy.

 

Combination with calcium antagonists, Class I antiarrhytmic drugs and centrally acting antihypertensive drugs:

Combination of bisoprolol with calcium antagonists of the verapamil or diltiazem type, with Class I antiarrhytmic drugs and with centrally acting antihypertensive drugs is generally not recommended see section 4.5)

 

Stopping treatment:

Abrupt cessation of therapy with a beta-blocker should be avoided, especially in patients with ischaemic heart disease, because this may lead to transitional worsening of heart condition. The posology should be decreased gradually, using the individual components, ideally over a period of two weeks while at the same time starting the replacement therapy if necessary.

 

Bradycardia:

If, during treatment, resting heart rate drops below 50-55 beats per minute and the patient experiences symptoms related to bradycardia, Cosyrel dose should be downtitrated using the individual components with an appropriate dose of bisoprolol.

 

First degree AV block:

Given their negative dromotropic effect, beta-blockers should be administered with caution to patients with first degree AV block.

 

Aortic and mitral valve stenosis / hypertrophic cardiomyopathy:

As with other ACE inhibitors, perindopril should be given with caution to patients with mitral valve stenosis and obstruction in the outflow of the left ventricle such as aortic stenosis or hypertrophic cardiomyopathy.

 

Prinzmetal’s angina:

Cases of coronary vasospam have been observed. Despite its high beta 1-selectivity, angina attacks cannot be completely excluded when bisoprolol is administered to patients with Printzmetal’s angina.

 

Renal impairment:

In case of renal impairment, the daily dose of Cosyrel should be adjusted according to creatinine clearance (see section 4.2).Routine monitoring of potassium and creatinine are part of normal medical practice for these patients (see section 4.8).

In patients with symptomatic heart failure, hypotension following the initiation of therapy with ACE inhibitors may lead to some further impairment in renal function. Acute renal failure, usually reversible, has been reported in this situation.

In some patients with bilateral renal artery stenosis or stenosis of the artery to a solitary kidney, who have been treated with ACE inhibitors, increases in blood urea and serum creatinine, usually reversible upon discontinuation of therapy, have been seen. This is especially likely in patients with renal insufficiency. If renovascular hypertension is also present there is an increased risk of severe hypotension and renal insufficiency. In these patients, treatment should be started under close medical supervision with low doses and careful dose titration. Since treatment with diuretics may be a contributory factor to the above, they should be discontinued and renal function should be monitored during the first weeks of treatment therapy.

Some hypertensive patients with no apparent pre-existing renal vascular disease have developed increases in blood urea and serum creatinine, usually minor and transient, especially when perindopril has been given concomitantly with a diuretic. This is more likely to occur in patients with pre-existing renal impairment. Dosage reduction and/or discontinuation of the diuretic and/or perindopril may be required.

 

Renovascular hypertension:

There is an increased risk of hypotension and renal insufficiency when patient with bilateral renal artery stenosis or stenosis of the artery to a single functioning kidney are treated with ACE inhibitors (see section 4.3). Treatment with diuretics may be a contributory factor. Loss of renal function may occur with only minor changes in serum creatinine even in patients with unilateral renal artery stenosis.

 

Kidney transplantation:

There is no experience regarding the administration of perindopril arginine in patients with recent kidney transplantation.

 

Haemodialysis patients:

Anaphylactoid reactions have been reported in patients dialysed with high flux membranes, and treated concomitantly with an ACE inhibitor. In these patients consideration should be given to using a different type of dialysis membrane or different class of antihypertensive agent.

 

Anaphylactoid reactions during low-density lipoproteins (LDL) apheresis:

Rarely, patients receiving ACE inhibitors during LDL apheresis with dextran sulphate have experienced life-threatening anaphylactoid reactions. These reactions were avoided by temporarily withholding ACE inhibitor therapy prior to each apheresis.

 

Anaphylactic reactions during desensitisation:

Patients receiving ACE inhibitors during desensitisation treatment (e.g. hymenoptera venom) have experienced anaphylactoid reactions. In the same patients, these reactions have been avoided when the ACE inhibitors were temporarily withheld, but they reappeared upon inadvertent rechallenge.

As with other beta-blockers, bisoprolol may increase both the sensitivity towards allergens and the severity of anaphylactic reactions. Epinephrine treatment does not always yield the expected therapeutic effect.

 

Neutropenia/Agranulocytosis/Thrombocytopenia/Anaemia:

Neutropenia/agranulocytosis, thrombocytopenia and anaemia have been reported in patients receiving ACE inhibitors. In patients with normal renal function and no other complicating factors, neutropenia occurs rarely. Perindopril should be used with extreme caution in patients with collagen vascular disease, immunosuppressant therapy, treatment with allopurinol or procainamide, or a combination of these complicating factors, especially if there is pre-existing impaired renal function. Some of these patients developed serious infections, which in a few instances did not respond to intensive antibiotic therapy. If perindopril is used in such patients, periodic monitoring of white blood cell counts is advised and patients should be instructed to report any sign of infection (e.g. sore throat, fever).

 

Bronchospasm (Bronchial asthma,obstructive airways diseases):

In bronchial asthma or other chronic obstructive lung diseases, which may cause symptoms, bronchodilating therapy should be given concomitantly. Occasionally an increase of the airway resistance may occur when beta-blockers are used in patients with asthma, therefore the dose of beta2-stimulants may have to be increased.

 

Diabetic patients:

Caution is advised when Cosyrel is used in patients with diabetes mellitus with large fluctuations in blood glucose values. Symptoms of hypoglycaemia can be masked by beta-blockers.

 

Strict fasting:

Caution is advised in patients with strict fasting.

 

Peripheral arterial occlusive disease:

Aggravation of symptoms may occur with beta-blockers, especially when starting therapy.

 

Anaesthesia:

In patients undergoing general anaesthesia beta-blockade reduces the incidence of arrhythmias and myocardial ischemia during induction and intubation, and the post-operative period. It is currently recommended that maintenance beta-blockade be continued peri-operatively. The anaesthesist must be aware of beta-blockade because of the potential for interactions with other drugs, resulting in bradyarrhythmias, attenuation of the reflex tachycardia and the decreased reflex ability to compensate for blood loss. If it is thought necessary to withdraw beta-blocker therapy before surgery, this should be done gradually and completed about 48 hours before anaesthesia.

In patients undergoing major surgery or during anaesthesia with agents that produce hypotension, perindopril may block angiotensin II formation secondary to compensatory renin release. The treatment should be discontinued one day prior to the surgery. If hypotension occurs and is considered to be due to this mechanism, it can be corrected by volume expansion.

 

Psoriasis:

Patients with psoriasis or with a history of psoriasis should only be given beta-blockers after carefully balancing the benefits against the risks.

 

Phaeochromocytoma:

In patients with known or suspected to have phaeochromocytoma bisoprolol should always be given in combination with an alpha-receptor blocker.

 

Thyreotoxicosis:

Under treatment with bisoprolol the symptoms of a thyreotoxicosis may be masked.

 

Primary aldosteronism:

Patients with primary hyperaldosteronism generally will not respond to anti-hypertensive drugs acting through inhibition of the renin-angiotensin system. Therefore, the use of this product is not recommended.

 

Pregnancy:

Unless continued ACE inhibitor therapy is considered essential, patients planning pregnancy should be changed to alternative anti-hypertensive treatments which have an established safety profile for use in pregnancy. When pregnancy is diagnosed, treatment with ACE inhibitors should be stopped immediately, and, if appropriate, alternative therapy should be started (see sections 4.3 and 4.6).

 

Heart failure:

There is no therapeutic experience of bisoprolol treatment of heart failure in patients with the following diseases and conditions:

•       insulin dependent diabetes mellitus (type I),

•       severely impaired renal function,

•       severely impaired hepatic function,

•       restrictive cardiomyopathy,

•       congenital heart disease,

•       haemodynamically significant organic valvular disease,

myocardial infarction within the last 3 months.

 

Excipients:

Level of sodium

Cosyrel contains less than 1 mmol sodium (23 mg) per tablet, i.e. essentially ‘sodium-free’.


No interactions between bisoprolol and perindopril have been observed in an interaction study conducted in healthy volunteers. Only information on interactions with other products that are known for the individual active substances is provided below.

 

Drugs increasing the risk of angioedema:

Concomitant use of ACE inhibitors with sacubitril/valsartan is contraindicated as this increases the risk of angioedema (see section 4.3 and 4.4). Sacubitril/valsartan must not be started until 36 hours after taking the last dose of perindopril therapy. Perindopril therapy must not be started until 36 hours after the last dose of sacubitril/valsartan (see sections 4.3 and 4.4).

Concomitant use of ACE inhibitors with racecadotril, mTOR inhibitors (e.g. sirolimus, everolimus, temsirolimus) and gliptins (e.g. linagliptin, saxagliptin, sitagliptin, vildagliptin) may lead to an increased risk for angioedema (see section 4.4).

 

 

Drugs inducing hyperkalaemia:

Although serum potassium usually remains within normal limits, hyperkalaemia may occur in some patients treated with Cosyrel. Some drugs or therapeutic classes may increase the occurrence of hyperkalaemia: aliskiren, potassium salts, potassium-sparing diuretics (e.g. spironolactone, triamterene or amiloride), ACE inhibitors, angiotensin-II receptors antagonists, NSAIDs, heparins, immunosuppressant agents such as ciclosporin or tacrolimus, trimethoprim and cotrimoxazole (trimethoprim/sulfamethoxazole), as trimethoprim is known to act as a potassium-sparing diuretic like amiloride.. The combination of these drugs increases the risk of hyperkalaemia. Therefore, the combination of Cosyrel with the above-mentioned drugs is not recommended. If concomitant use is indicated, they should be used with caution and with frequent monitoring of serum potassium.

 

 

Concomitant use contraindicated (see section 4.3)

 

Aliskiren:

The concomitant therapy with Cosyrel and aliskiren is contra-indicated in diabetic or impaired renal patients, due to the risk of hyperkalaemia, worsening of renal function and cardiovascular morbidity and mortality increase.

 

Extracorporeal treatments:

Extracorporeal treatments leading to contact of blood with negatively charged surfaces such as dialysis or haemofiltration with certain high-flux membranes (e.g. polyacrylonitril membranes) and low density lipoprotein apheresis with dextran sulphate due to increased risk of severe anaphylactoid reactions (see section 4.3). If such treatment is required, consideration should be given to using a different type of dialysis membrane or a different class of antihypertensive agent.

 

Concomitant use not recommended

 

Linked to bisoprolol

 

Centrally acting antihypertensives such as clonidine and others (e.g. methyldopa, moxonidine, rilmenidine):

Concomitant use of centrally acting antihypertensives may worsen heart failure by lowering the central sympathetic tonus (reduced heart rate and cardiac output, vasodilation). Abrupt termination, particularly before down-titration of beta-blocker therapy, may increase the risk of rebound hypertension.

 

Class I antiarrhythmic drugs (e.g. quinidine, disopyramide; lidocaine, phenytoin; flecainide, propafenone):

Effect on atrio-ventricular conduction time may be potentiated and negative inotropic effect increased.

 

Calcium antagonists of the verapamil type and to a lesser extent of the diltiazem type:

Negative influence on contractility and atrio-ventricular conduction. Intravenous administration of verapamil in patients on beta-blocker treatment may lead to profound hypotension and atrio-ventricular block.

 

Linked to perindopril

 

Aliskiren:

In patients other than diabetic or impaired renal patients, risk of hyperkalaemia, worsening of renal function and cardiovascular morbidity and mortality increase.

 

Concomitant therapy with ACE inhibitor and angiotensin-receptor blocker:

Clinical trial data has shown that dual blockade of the renin-angiotensin-aldosterone-system (RAAS) through the combined use of ACE-inhibitors, angiotensin II receptor blockers or aliskiren is associated with a higher frequency of adverse events such as hypotension, hyperkalaemia and decreased renal function (including acute renal failure) compared to the use of a single RAAS-acting agent (see sections 4.3, 4.4 and 5.1).

It has been reported in the literature that in patients with established atherosclerotic disease, heart failure, or with diabetes with end organ damage, concomitant therapy with ACE inhibitor and angiotensin-receptor blocker is associated with a higher frequency of hypotension, syncope, hyperkalaemia, and worsening renal function (including acute renal failure) as compared to use of a single renin-angiotensin-aldosterone system agent. Dual blockade (e.g, by combining an ACE inhibitor with an angiotensin II receptor antagonist) should be limited to individually defined cases with close monitoring of renal function, potassium levels, and blood pressure.

 

Estramustine:

Risk of increased adverse effects such as angioneurotic oedema (angioedema).

 

 

Potassium sparing diuretics (e.g. triamterene, amiloride...), potassium (salts):

Hyperkalaemia (potentially lethal), especially in conjunction with renal impairment (additive hyperkalaemic effects).

 

The combination of perindopril with the above-mentioned drugs is not recommended (see section 4.4). If concomitant use is nonetheless indicated they should be used with caution and with frequent monitoring of serum potassium. For use of spironolactone in heart failure, see below.

 

Lithium:

Reversible increases in serum lithium concentrations and toxicity have been reported during concomitant administration of lithium with ACE inhibitors. Use of perindopril with lithium is not recommended, but if the combination proves necessary, careful monitoring of serum lithium levels should be performed (see section 4.4).

 

Concomitant use which require special care

 

Linked to bisoprolol and perindopril

 

Antidiabetic agents (insulins, oral hypoglycaemic agents):

Epidemiological studies have suggested that concomitant administration of ACE inhibitors and antidiabetic medicines (insulins, oral hypoglycaemic agents) may cause an increased blood-glucose lowering effect with risk of hypoglycaemia. This phenomenon appeared to be more likely to occur during the first weeks of combined treatment and in patients with renal impairment.

Concomitant administration of bisoprolol with insulin and oral antidiabetic drugs may increase blood sugar lowering effect. Blockade of beta-adrenoreceptors may mask symptoms of hypoglycaemia.

 

Non-steroidal anti-inflammatory medicinal products (NSAIDs) (including acetylsalicylic acid ≥ 3 g/day):

The administration of Cosyrel simultaneously with non-steroidal anti-inflammatory drugs (i.e. acetylsalicylic acid at anti-inflammatory dosage regimens, COX-2 inhibitors and non-selective NSAIDs) may attenuate the antihypertensive effect of bisoprolol and perindopril.

In addition, concomitant use of ACE inhibitors and NSAIDs may lead to an increased risk of worsening of renal function, including possible acute renal failure, and an increase in serum potassium, especially in patients with poor pre-existing renal function. The combination should be administered with caution, especially in the elderly. Patients should be adequately hydrated and consideration should be given to monitoring renal function after initiation of concomitant therapy, and periodically thereafter.

 

Antihypertensive agents and vasodilators:

Concomitant use with antihypertensive agents, vasodilators (such as nitroglycerin, other nitrates or other vasodilators) or with other medications which have a blood-pressure-reducing potential (e.g. tricyclic antidepressants, barbiturates, phenothiazines) may increase the risk of hypotensive effects of perindopril and bisoprolol.

 

Tricyclic antidepressants/Antipsychotics/Anesthetics:

Concomitant use of ACE inhibitors with certain anaesthetic medicinal products, tricyclic antidepressants and antipsychotics may result in further reduction of blood pressure.

Concomitant use of bisoprolol with anaesthesics may lead to reduced reflex tachycardia and increased risk of hypotension.

 

Sympathomimetics:

Beta-sympathomimetics (e.g. isoprenaline, dobutamine): combination with bisoprolol may reduce the effects of both agents.

Sympathomimetics that activate both beta- and alpha-adrenoceptors (e.g. norepinephrine, epinephrine): combination with bisoprolol may unmask the alpha-adrenoceptor-mediated vasoconstrictor effects of these agents, leading to blood pressure increase and exacerbated intermittent claudication. Such interactions are considered to be more likely with nonselective beta-blockers.

Sympathomimetics may reduce the antihypertensive effects of ACE inhibitors.

 

Linked to bisoprolol

 

Calcium antagonists of the dihydropyridine type such as felodipine and amlodipine:

Concomitant use may increase the risk of hypotension, and an increase in the risk of a further deterioration of the ventricular pump function in patients with heart failure cannot be excluded.

 

Class-III antiarrhythmic drugs (e.g. amiodarone):

Effect on atrio-ventricular conduction time may be potentiated.

 

Parasympathomimetic drugs:

Concomitant use may increase atrio-ventricular conduction time and the risk of bradycardia.

 

Topical beta-blockers (e.g. eye drops for glaucoma treatment):

Concomitant use may add to the systemic effects of bisoprolol.

Digitalis glycosides:

Reduction of heart rate, increase of atrio-ventricular conduction time.

 

Linked to perindopril

 

Baclofen:

Increased antihypertensive effect. Monitor blood pressure and adapt antihypertensive dosage if necessary.

 

Non-potassium-sparing diuretics:

Patients on diuretics, and especially those who are volume and/or salt depleted, may experience excessive reduction in blood pressure after initiation of therapy with an ACE inhibitor. The possibility of hypotensive effects can be reduced by discontinuation of the diuretic, by increasing volume or salt intake prior to initiating therapy with low and progressive doses of perindopril.

In arterial hypertension, when prior diuretic therapy can have caused salt/volume depletion, either the diuretic must be discontinued before initiating the ACE inhibitor, in which case a non-potassium-sparing diuretic can be thereafter reintroduced or the ACE inhibitor must be initiated with a low dosage and progressively increased.

In diuretic-treated congestive heart failure, the ACE inhibitor should be initiated at a very low dosage, possibly after reducing the dosage of the associated non-potassium-sparing diuretic.

In all cases, renal function (creatinine levels) must be monitored during the first few weeks of ACE inhibitor therapy.

 

Potassium-sparing diuretics (eplerenone, spironolactone):

With eplerenone or spironolactone at doses between 12,5 mg to 50 mg by day and with low doses of ACE inhibitors:

In the treatment of class II-IV heart failure (NYHA) with an ejection fraction < 40%, and previously treated with ACE inhibitors and loop diuretics, risk of hyperkalaemia, potentially lethal, especially in case of non-observance of the prescription recommendations on this combination.

Before initiating the combination, check the absence of hyperkalaemia and renal impairment.

A close monitoring of the kalaemia and creatininemia is recommended in the first month of the treatment once a week at the beginning and, monthly thereafter.

Mefloquine:

Increased risk of bradycardia.

 

Monoamine oxidase inhibitors (except MAO-B inhibitors):

Enhanced hypotensive effect of the beta-blockers but also risk for hypertensive crisis.

 

Linked to perindopril

 

 

Gold:

Nitritoid reactions (symptoms include facial flushing, nausea, vomiting and hypotension) have been reported rarely in patients on therapy with injectable gold (sodium aurothiomalate) and concomitant ACE inhibitor therapy including perindopril.


Pregnancy:

Based on existing data on monocomponents, Cosyrel is not recommended during the first trimester of pregnancy and is contraindicated during the second and third trimesters of pregnancy.

 

Bisoprolol

 

Bisoprolol has pharmacological effects that may cause harmful effects on pregnancy and/or the foetus/newborn (reduce placental perfusion associated with growth retardation, intrauterine death, abortion or early labour and adverse effects (e.g. hypoglycaemia and bradycardia) may occur in the foetus and newborn infant). If treatment with beta-adrenoceptor blockers is necessary, beta-1-selective adrenoceptor blockers are preferable.

Bisoprolol should not be used during pregnancy unless clearly necessary. If treatment with bisoprolol is considered necessary, the uteroplacental blood flow and the foetal growth should be monitored. In case of harmful effects on pregnancy or the foetus alternative treatment should be considered. The newborn infant must be closely monitored.

Symptoms of hypoglycaemia and bradycardia are generally to be expected within the first 3 days.

 

Perindopril

 

Epidemiological evidence regarding the risk of teratogenicity following exposure to ACE inhibitors during the first trimester of pregnancy has not been conclusive; however a small increase in risk cannot be excluded. Unless continued ACE inhibitor therapy is considered essential, patients planning pregnancy should be changed to alternative anti-hypertensive treatments which have an established safety profile for use in pregnancy. When pregnancy is diagnosed, treatment with ACE inhibitors should be stopped immediately, and, if appropriate, alternative therapy should be started.

Exposure to ACE inhibitor therapy during the second and third trimesters is known to induce human foetotoxicity (decreased renal function, oligohydramnios, skull ossification retardation) and neonatal toxicity (renal failure, hypotension, hyperkalaemia) (see section 5.3). Should exposure to ACE inhibitor have occurred from the second trimester of pregnancy, ultrasound check of renal function and skull is recommended. Infants whose mothers have taken ACE inhibitors should be closely observed for hypotension (see also sections 4.3 and 4.4).

 

Breast-feeding:

Cosyrel is not recommended during lactation.

 

It is not known whether bisoprolol is excreted in human milk. Therefore, breastfeeding is not recommended during administration of bisoprolol.

Because no information is available regarding the use of perindopril during breastfeeding, perindopril is not recommended and alternative treatments with better established safety profiles during breast-feeding are preferable, especially while nursing a newborn or preterm infant.

 

Fertility:

There are no clinical data on fertility with the use of Cosyrel.


Cosyrel has no direct influence on the ability to drive and use machines but individual reactions related to low blood pressure may occur in some patients, particularly at the start of treatment or upon change of medication as well as in conjunction with alcohol.

As a result the ability to drive or operate machinery may be impaired.

 


Summary of the safety profile:

The most common adverse reactions to bisoprolol include headache, dizziness, worsening of heart failure, hypotension, cold extremities, nausea, vomiting, abdominal pain, diarrhoea, constipation, asthenia and fatigue.

The most common adverse reactions reported in clinical trials and observed with perindopril include headache, dizziness, vertigo, paraesthesia, visual disturbance, tinnitus, hypotension, cough, dyspnoea, nausea, vomiting, abdominal pain, diarrhoea, constipation, dysgeusia, dyspepsia, rash, pruritus, muscle cramps and asthenia.

There is no information on overdose with Cosyrel in humans.

 

Bisoprolol

 

Symptoms:

In general the most common signs expected with overdosage of a beta-blocker are bradycardia, hypotension, bronchospasm, acute cardiac insufficiency and hypoglycaemia. To date a few cases of overdose (maximum: 2000 mg) with bisoprolol have been reported in patients suffering from hypertension and/or coronary heart disease showing bradycardia and/or hypotension; all patients recovered. There is a wide interindividual variation in sensitivity to one single high dose of bisoprolol and patients with heart failure are probably very sensitive.

 

Management:

If overdose occurs, bisoprolol treatment should be stopped and supportive and symptomatic treatment should be provided. Limited data suggest that bisoprolol is hardly dialysable. Based on the expected pharmacologic actions and recommendations for other beta-blockers, the following general measures should be considered when clinically warranted.

Bradycardia: Administer intravenous atropine. If the response is inadequate, isoprenaline or another agent with positive chronotropic properties may be given cautiously. Under some circumstances, transvenous pacemaker insertion may be necessary.

Hypotension: Intravenous fluids and vasopressors should be administered. Intravenous glucagon may be useful.

AV block (second or third degree): Patients should be carefully monitored and treated with isoprenaline infusion or transvenous cardiac pacemaker insertion.

Acute worsening of heart failure: Administer i.v. diuretics, inotropic agents, vasodilating agents.

Bronchospasm: Administer bronchodilator therapy such as isoprenaline, beta2-sympathomimetic drugs and/or aminophylline.

Hypoglycaemia: Administer i.v. glucose.

 

Perindopril

 

Symptoms:

Limited data are available for overdosage in humans. Symptoms associated with overdosage of ACE inhibitors may include hypotension, circulatory shock, electrolyte disturbances, renal failure, hyperventilation, tachycardia, palpitations, bradycardia, dizziness, anxiety, and cough.

 

Management:

The recommended treatment of overdosage is intravenous infusion of sodium chloride 9 mg/ml (0.9%) solution. If hypotension occurs, the patient should be placed in the shock position. If available, treatment with angiotensin II infusion and/or intravenous catecholamines may also be considered. Perindopril may be removed from the general circulation by haemodialysis (see section 4.4). Pacemaker therapy is indicated for therapy-resistant bradycardia. Vital signs, serum electrolytes and creatinine concentrations should be monitored continuously.

 

 


5.1     Pharmacodynamic properties

Mechanism of action

 

Bisoprolol

 

Bisoprolol is a highly beta1-selective-adrenoceptor blocking agent, lacking intrinsic stimulating and relevant membrane stabilising activity. It only shows low affinity to the beta2-receptor of the smooth muscles of bronchi and vessels as well as to the beta2-receptors concerned with metabolic regulation. Therefore, bisoprolol is generally not to be expected to influence the airway resistance and beta2-mediated metabolic effects. Its beta1-selectivity extends beyond the therapeutic dose range.

 

Perindopril

 

Perindopril is an inhibitor of the enzyme that converts angiotensin I into angiotensin II (ACE). The converting enzyme, or kininase, is an exopeptidase that allows conversion of angiotensin I into the vasoconstrictor angiotensin II as well as causing the degradation of the vasodilator bradykinin into an inactive heptapeptide. Inhibition of ACE results in a reduction of angiotensin II in the plasma, which leads to increased plasma renin activity (by inhibition of the negative feedback of renin release) and reduced secretion of aldosterone. Since ACE inactivates bradykinin, inhibition of ACE also results in an increased activity of circulating and local kallikrein-kinin systems (and thus also activation of the prostaglandin system). It is possible that this mechanism contributes to the blood pressure-lowering action of ACE inhibitors and is partially responsible for certain of their side effects (e.g. cough).

Perindopril acts through its active metabolite, perindoprilat. The other metabolites show no inhibition of ACE activity in vitro.

 

Pharmacodynamic effects

 

Bisoprolol

 

Bisoprolol has no significant negative inotropic effects.

Bisoprolol reaches its maximum effects 3-4 hours after administration. Due to the half-life of 10-12 hours, bisoprolol acts for 24 hours.

The maximum blood-pressure-lowering effects of bisoprolol are generally reached after 2 weeks.

 

In acute administration in patients with coronary heart disease without chronic heart failure bisoprolol reduces the heart rate and stroke volume and thus the cardiac output and oxygen consumption. In chronic administration the initially elevated peripheral resistance decreases. The decrease in plasma renin activity is proposed as a mechanism of action underlying the antihypertensive effect of beta-blockers.

 

Bisoprolol reduces the sympatho-adrenergic response by blocking cardiac beta-adrenergic receptors. This results in a decrease in heart rate and contractility, causing a reduction in oxygen consumption by the myocardium, which is the desired effect in the case of angina associated with underlying coronary heart disease.

 

Perindopril

 

Hypertension:

Perindopril is active in all grades of hypertension: mild, moderate, severe; a reduction in systolic and diastolic blood pressures in both supine and standing positions is observed.

Perindopril reduces peripheral vascular resistance, leading to blood pressure reduction. As a consequence, peripheral blood flow increases, with no effect on heart rate.

Renal blood flow increases as a rule, while the glomerular filtration rate (GFR) is usually unchanged.

 

Clinical efficacy and safety

 

Bisoprolol

 

In total 2647 patients were included in the CIBIS II trial. 83% (n = 2202) were in NYHA class III and 17% (n = 445) were in NYHA class IV. They had stable symptomatic systolic heart failure (ejection fraction <35%, based on echocardiography). Total mortality was reduced from 17.3% to 11.8% (relative reduction 34%). A decrease in sudden death (3.6% vs 6.3%, relative reduction 44%) and a reduced number of heart failure episodes requiring hospital admission (12% vs 17.6%, relative reduction 36%) was observed. Finally, a significant improvement of the functional status according to NYHA classification has been shown. During the initiation and titration of bisoprolol hospital admission due to bradycardia (0.53%), hypotension (0.23%), and acute decompensation (4.97%) were observed, but they were not more frequent than in the placebo-group (0%, 0.3% and 6.74%). The numbers of fatal and disabling strokes during the total study period were 20 in the bisoprolol group and 15 in the placebo group.

 

The CIBIS III trial investigated 1010 patients aged ≥65 years with mild to moderate chronic heart failure (CHF; NYHA class II or III) and left ventricular ejection fraction ≤35%, who had not been treated previously with ACE inhibitors, beta-blockers, or angiotensin receptor blockers. Patients were treated with a combination of bisoprolol and enalapril for 6 to 24 months after an initial 6 months treatment with either bisoprolol or enalapril.

There was a trend toward higher frequency of chronic heart failure worsening when bisoprolol was used as the initial 6 months treatment. Non inferiority of bisoprolol-first versus enalapril-first treatment was not proven in the per-protocol analysis, although the two strategies for initiation of CHF treatment showed a similar rate of the primary combined endpoint death and hospitalization at study end (32.4% in the bisoprolol-first group vs. 33.1 % in the enalapril-first group, per-protocol population). The study shows that bisoprolol can also be used in elderly chronic heart failure patients with mild to moderate disease.

 

Perindopril

 

Hypertension:

Perindopril is active in all grades of hypertension: mild, moderate, severe; a reduction in systolic and diastolic blood pressures in both supine and standing positions is observed.

Perindopril reduces peripheral vascular resistance, leading to blood pressure reduction. As a consequence, peripheral blood flow increases, with no effect on heart rate.

Renal blood flow increases as a rule, while the GFR is usually unchanged.

The antihypertensive activity is maximal between 4 and 6 hours after a single dose and is sustained for at least 24 hours: trough effects are about 87- 100 % of peak effects.

The decrease in blood pressure occurs rapidly. In responding patients, normalisation is achieved within a month and persists without the occurrence of tachyphylaxis.

Discontinuation of treatment does not lead to a rebound effect.

Perindopril reduces left ventricular hypertrophy.

In man, perindopril has been confirmed to demonstrate vasodilatory properties. It improves large artery elasticity and decreases the media: lumen ratio of small arteries.

An adjunctive therapy with a thiazide diuretic produces an additive-type of synergy. The combination of an ACE inhibitor and a thiazide also decreases the risk of hypokalaemia induced by the diuretic treatment.

 

Patients with stable coronary artery disease:

The EUROPA study was a multicentre, international, randomised, double-blind, placebo-controlled clinical trial lasting 4 years.

Twelve thousand two hundred and eighteen (12 218) patients aged over 18 were randomised to 8 mg perindopril tert-butylamine (equivalent to 10 mg perindopril arginine) (n = 6 110) or placebo (n = 6 108).

The trial population had evidence of coronary artery disease with no evidence of clinical signs of heart failure. Overall, 90% of the patients had a previous myocardial infarction and/or a previous coronary revascularisation. Most of the patients received the study medication on top of conventional therapy including platelet inhibitors, lipid lowering agents and beta-blockers.

The main efficacy criterion was the composite of cardiovascular mortality, non-fatal myocardial infarction and/or cardiac arrest with successful resuscitation. The treatment with 8 mg perindopril tert-butylamine (equivalent to 10 mg perindopril arginine) once daily resulted in a significant absolute reduction in the primary endpoint of 1.9% (relative risk reduction of 20%, 95%CI [9.4; 28.6] – p<0.001).

In patients with a history of myocardial infarction and/or revascularisation, an absolute reduction of 2.2% corresponding to a RRR of 22.4% (95%CI [12.0; 31.6] – p<0.001) in the primary endpoint was observed by comparison to placebo.

 

In a subgroup of patients treated with beta-blockers from EUROPA study defined in a post-hoc analysis, the addition of perindopril to beta-blockers (n=3789) showed a significant absolute reduction of 2.2% (relative risk reduction of 24%, 95%CI [9.5; 36.4]) compared to beta-blockers without perindopril (n=3745) in the composite of cardiovascular mortality, non-fatal myocardial infarction and/or cardiac arrest with successful resuscitation.

 

Dual blockade of the renin-angiotensin-aldosterone system (RAAS) clinical trial data:

Two large randomised, controlled trials (ONTARGET (ONgoing Telmisartan Alone and in combination with Ramipril Global Endpoint Trial) and VA NEPHRON-D (The Veterans Affairs Nephropathy in Diabetes) have examined the use of combination of an ACE-inhibitor with an angiotensin II receptor blocker.

ONTARGET was a study conducted in patients with a history of cardiovascular or cerebrovascular disease, or type 2 diabetes mellitus accompanied by evidence of end-organ damage. VA NEPHRON-D was a study in patients with type 2 diabetes mellitus and diabetic nephropathy.

These studies have shown no significant beneficial effect on renal and/or cardiovascular outcomes and mortality, while an increased risk of hyperkalaemia, acute kidney injury and/or hypotension as compared to monotherapy was observed. Given their similar pharmacodynamic properties, these results are also relevant for other ACE-inhibitors and angiotensin II receptor blockers.

ACE-inhibitors and angiotensin II receptor blockers should therefore not be used concomitantly in patients with diabetic nephropathy.

ALTITUDE (Aliskiren Trial in Type 2 Diabetes Using Cardiovascular and Renal Disease Endpoints) was a study designed to test the benefit of adding aliskiren to a standard therapy of an ACE-inhibitor or an angiotensin II receptor blocker in patients with type 2 diabetes mellitus and chronic kidney disease, cardiovascular disease, or both. The study was terminated early because of an increased risk of adverse outcomes. Cardiovascular death and stroke were both numerically more frequent in the aliskiren group than in the placebo group and adverse events and serious adverse events of interest (hyperkalaemia, hypotension and renal dysfunction) were more frequently reported in the aliskiren group than in the placebo group.

 

Paediatric population:

No data are available with Cosyrel in children.

The European Medicines Agency has granted a product-specific waiver for Cosyrel in all subsets of the paediatric population in the treatment of hypertension, ischaemic coronary artery disease stable and chronic heart failure (see section 4.2 for information on paediatric use).

 


The rate and extent of absorption of bisoprolol and perindopril from Cosyrel are not significantly different, respectively, from the rate and extent of absorption of bisoprolol and perindopril when taken alone as monotherapy.

 

Bisoprolol

 

Absorption

Bisoprolol is almost completely (>90%) absorbed from the gastrointestinal tract and, because of its small hepatic first-pass metabolism (approximately 10%), it has a bioavailability of approximately 90% after oral administration.

 

Distribution

The distribution volume is 3.5 l/kg. The plasma protein binding of bisoprolol is about 30%.

 

Biotransformation and elimination

Bisoprolol is excreted from the body by two routes. 50% is metabolised by the liver to inactive metabolites which are then excreted by the kidneys. The remaining 50% is excreted by the kidneys in an unmetabolised form. Total clearance is approximately 15 l/h. The half-life in plasma of 10-12 hours gives a 24 hour effect after dosing once daily.

 

Special populations

The kinetics of bisoprolol are linear and independent of age.

Since the elimination takes place in the kidneys and the liver to the same extent a dosage adjustment is not required for patients with impaired liver function or renal insufficiency. The pharmacokinetics in patients with chronic heart failure and with impaired liver or renal function has not been studied. In patients with chronic heart failure (NYHA stage III) the plasma levels of bisoprolol are higher and the half-life is prolonged compared to healthy volunteers. Maximum plasma concentration at steady state is 64±21 ng/ml at a daily dose of 10 mg and the half-life is 17±5 hours.

 

Perindopril

 

Absorption

After oral administration, the absorption of perindopril is rapid and the peak concentration is achieved within 1 hour. The plasma half-life of perindopril is equal to 1 hour.

 

Distribution

The volume of distribution is approximately 0.2 l/kg for unbound perindoprilat. Protein binding of perindoprilat to plasma proteins is 20%, principally to ACE, but is concentration-dependent.

 

Biotransformation

Perindopril is a prodrug. Twenty seven percent of the administered perindopril dose reaches the bloodstream as the active metabolite perindoprilat. In addition to active perindoprilat, perindopril yields five metabolites, all inactive. The peak plasma concentration of perindoprilat is achieved within 3 to 4 hours.

As ingestion of food decreases conversion to perindoprilat, hence bioavailability, perindopril arginine should be administered orally in a single daily dose in the morning before a meal.

 

Elimination

Perindoprilat is eliminated in the urine and the terminal half-life of the unbound fraction is approximately 17 hours, resulting in steady-state within 4 days.

 

Linearity

It has been demonstrated a linear relationship between the dose of perindopril and its plasma exposure.

 

Special population:

Elimination of perindoprilat is decreased in the elderly, and also in patients with heart or renal failure. Dosage adjustment in renal insufficiency is desirable depending on the degree of impairment (creatinine clearance).

Dialysis clearance of perindoprilat is equal to 70 ml/min.

Perindopril kinetics are modified in patients with cirrhosis: hepatic clearance of the parent molecule is reduced by half. However, the quantity of perindoprilat formed is not reduced and therefore no dosage adjustment is required (see sections 4.2 and 4.4).

 


Bisoprolol

 

Preclinical data reveal no special hazard for humans based on conventional studies of safety pharmacology, repeated dose toxicity, genotoxicity or carcinogenicity.

In reproductive toxicology studies, bisoprolol had no effect on fertility or other general results of reproduction.

Like other beta-blockers, bisoprolol caused maternal (decreased food intake and decreased body weight) and embryo/fetal toxicity (increased incidence of resorptions, reduced birth weight of the offspring, retarded physical development) at high doses but was not teratogenic.

 

Perindopril

 

In the chronic oral toxicity studies (rats and monkeys), the target organ is the kidney, with reversible damage.

No mutagenicity has been observed in in vitro or in vivo studies.

Reproduction toxicology studies (rats, mice, rabbits and monkeys) showed no sign of embryotoxicity or teratogenicity. However, ACE inhibitors, as a class, have been shown to induce adverse effects on late foetal development, resulting in foetal death and congenital effects in rodents and rabbits: renal lesions and an increase in peri- and postnatal mortality have been observed. Fertility was not impaired either in male or in female rats

No carcinogenicity has been observed in long term studies in rats and mice.

 

Environmental Risk Assessment:

Cosyrel contains known active substances, bisoprolol and perindopril. Cosyrel will be prescribed as a direct replacement for individual doses of bisoprolol and perindopril, so there will be no increase in the environmental exposure.
